摘要
本研究主要針對某台北市立醫院輸尿管結石病患，依其接受不同治療的方式，進行疾病治療的成本與效果分析。
本研究為一回溯性研究，採立意取樣的方式，首先至醫院收集相關財務資料及醫療記錄，以獲得疾病治療的直接成本資料及客觀治療效果資料。並對患者進行電話訪問，以獲得疾病治療間接成本資料及主觀治療效果資料。
資料的分析包括描述性及分析性統計。玆將重要結果摘要如下：
一、結石治療成本
傳統手術及輸尿管鏡截石術，主要成本來自於事務成本，而體外震波碎石治療則來自於材料設備成本。輸尿管結石單次治療的成本(傳統手術組因個案數太少未列入比較) ，以輸尿管鏡截石術低於門診體外震波碎石術。然而結石治療成本，各組間差異均未達統計上顯著水準。單次治療及結石治療的費用，以輸尿管鏡截石術最低，其次為住院震波碎石術，而以門診震波碎石術最高。而病患及家屬生產力損失天數則以輸尿管鏡截石術組高於住院體外震波碎石術組再高於門診體外震波碎石術組。
二、結石治療效果
輸尿管結石清除率、症狀改善程度及治療滿意度，各組間差異均未達統計上顯著程度(傳統手術組因個案數太少未列入比較)。
三、對治療效果做多變項迥歸分析
較小的結石、下段輸尿管結石相較於上段輸尿管結石、輸尿管鏡截石術相較於門診體外震波碎石術，治療成本較低。輸尿管鏡截石術相較於住院體外震波碎石術及門診體外震波碎石術，治療支付費用較低。病患為男性、具有較大的結石、接受輸尿管鏡截石術治療相較於住院體外震波碎石術及門診體外震波碎石術，對於治療的滿意度較低。
本研究並依研究結果，分別對未來研究者、醫療機構管理者、及衛生主管機關提出建議，供有關單位參考。

Abstract
The purpose of this study was to evaluate the cost-effectiveness of four different ureteral stone treatment programs (OP: conventional operation; URS: ureteroscopic lithotripsy; Admitted ESWL: admitted extrocorporeal shock wave lithotripsy; OPD ESWL: out patient extrocoporeal lithotripsy), which were implemented in a Taipei municipal hospital.
This was a retrospective study. Medical records and hospital financial records were collected for costs of implementing each treatment program and the objective effectiveness data. The indirect cost of disease and the subjective effectives data were collected by telephone interviews. Data analysis included descriptive and inferential statistics. Important results are as follows:
1. The cost of stone treatment
Major costs of OP and URS come from general affairs, but that of ESWL was from equipment facilities. The single treatment cost of URS was lower than that of OPD ESWL. However, in terms of stone treatment, there is no significant difference between groups. In terms of the indirect costs, such as patient and family workday loss, URS group ranked the highest while OPD ESWL group was the lowest.
2. The effectiveness of stone treatment
In terms of stone clearness, symptom improvement and patient satisfaction, there was no significant difference between groups.
3. Multiple regression analysis of cost and effectiveness
In terms of treatment cost: smaller stone, lower ureteral stone to upper ureteral stone, and URS to OPD ESWL cost less. In terms of treatment charge: URS to Admitted ESWL and OPDESWL charge less. In terms of patient satisfaction: male, larger stone, and URS to Admitted ESWL and OPDESWL are less satisfied.
Based on the results, we propose suggestions to future researchers, hospital managers and health authorities.
