背景：白血病屬於重大傷病的一種，罹患人數逐年上升，病患一旦被確立診斷為白血病，需接受積極和長期的治療，病情緩解後必須規律地追蹤，接受週期性的維持治療以預防病情復發，所使用的藥物及治療方法都非常昂貴，對於病患家庭經濟及健保財務都是一項沈重的負擔。遍尋國內學術研究標的，對於白血病的醫療資源利用研究較少，因此有必要進一步探討，提供臨床醫療單位、醫院管理者及衛生政策決策者在未來制訂相關政策之參考。
方法：本研究是選取北區分局2004及2005年兩年之門診及住院病人，診斷中曾出現白血病者（即ICD-9-CM主診斷代碼為204至208）。並引用中央健保局北區分局2004及2005年之住院與門診醫療費用明細檔，以及醫療事業機構基本資料檔。統計分析工具為SPSS for windows 12.0版套裝軟體及決策樹，包括描述性統計、分類廻歸樹狀法，CART(Classification and Regression Tree)、卡方檢定、Pearson Correlation。
結果：研究結果顯示白血病之醫療資源利用多又分歧，且其差異非常大，以骨髓移植之醫療資源利用最高；而影響白血病醫療資源利用的主要因素有骨髓移植、合併症和併發症、年齡以及疾病型態等。
建議：研究發現白血病病人的醫療資源利用可以分成許多相似的群組，每一群組之間的差異很大，所以不妨以「論人計酬」方式給付，並伴以「個案管理」做為引導工具。因此若設計合適的給付方式，不僅與照顧的效益及品質有關，也和健保的支出息息相關。又每一組內資源耗用相當接近，故醫院應加以研究各組內有否差異，並考量將分組病人集中在某一家專科醫院醫治管理，採區域化醫療，達到經濟規模，如此或能降低醫療成本。

Abstract

Purpose: Leukemia is one of the National Health Insurance major illnesses. The number of patient suffered from leukemia increased rapidly in past recent years. Due to longtime progress and need of expensive drugs and treatments, the medical resource utilization was extremely high. Due to the shortage of research on patients with leukemia, the goal of this study is to explore the factors affecting the utilization and to propose some suggestions to pertaining administrative management.

Method: The research was to evaluate the characteristics of medical resource utilization in care of outpatients and inpatients with leukemia (ICD-9-CM code 204-208). The medical expenditure and other variables were extracted from Taipei Branch of the Bureau of National Health Insurance databank from 2004 to 2005. Those data were analyzed by SPSS for windows 12.0, using descriptive statistics, CART (Classification and regression Tree), chi-square test, Pearson correlation.

Result: Medical resource consumption revealed significant diversity and difference in many items. The major factors would influence the utilization were bone marrow transplantation, complications and comobidities, age and disease patterns.

Conclusion: Patients with Leukemia could be divided into different groups according to their expenditure. Each group has significant difference, capitation payment is suggested and case management is feasible. In addition, patients of same group would be treated at same facility to control the utilization and to reduce the medical cost.
