背景：台灣地區人口老化，每年國人十大死亡原因排列仍以慢性疾病為主，除了疾病的醫療服務外，更需要多元的長期照顧服務。因此瞭解出院病人與其主要照顧者的出院需求與需求滿足程度是重要的議題。
研究目的：本研究旨在探討骨科出院病人與其主要照顧者的出院需求與需求滿足程度。
研究方法：本研究為橫斷性之調查研究，研究對象為屏東某區域醫院骨科出院病人與其主要照顧者，利用結構式問卷，面談及電訪進行資料蒐集。研究資料以IBM SPSS 18.0統計軟體進行敘述性統計、獨立樣本t檢定、單因子變異數分析、成對樣本t檢定、多元迴歸等分析。
研究結果：(一)共有91位骨科出院病人與其主要照顧者參與本研究，出院病人平均年齡63.53歲，63.7%為女性，以不識字佔70.3%，38.5%為股骨頸骨折，50.5%行開放性復位(ORIF)，82.4%住院時身上有管路，52.7%有共病。主要照顧者平均年齡48.26歲，71.4%為女性，68.1%國高中學歷，83.5%信奉佛道教，31.9%為病患兒女，45.1%自覺健康為好，76.9%無類似照顧經驗。(二)在出院需求方面，整體出院需求平均11.15分，佔總量表的46.4%。出院需求整體平均得分較高的影響因素是病人有入住加護病房、主要照顧者自覺健康普通。(三)一週後出院需求滿足程度方面，整體需求滿足程度平均25.70分。一週後出院需求滿足程度整體平均得分較高的影響因素是病人有入住加護病房、主要照顧者年齡越大。(四) 一個月後出院需求滿足程度方面，整體需求滿足程度平均33.77分。一個月後出院需求滿足程度整體平均得分較高的影響因素是病人有入住加護病房。
研究結論：病人有入住加護病房、主要照顧者的年齡和自覺健康狀態是骨科出院病人與其主要照顧者的出院需求與需求滿足程度的主要影響因素。

Background: Though the population in Taiwan is aging, the ten major cause of death are still related to chronic diseases. Other than the treatment of the disease, what is needed more is various type of long term care service. Therefore, thorough understanding of the needs of the patients and their primary caretaker and satisfying them for after being released from the hospital is an import issue.
Research Purpose: This research is to examine the needs of the orthopedic patients and their primary caretaker and their satisfaction level of the results provided.
Methodology: This is a cross-sectional research design, the subjects are orthopedic patients and their primary caregivers in one regional hospital located in Pin-Dong. The data was collected through structural surveys and conducting interviews through face-to-face and through telephone. Data was analyzed with IBM SPSS 18.0 conducting descriptive statistic, T-test analysis, one-way ANOVA, paired-sample T-test and multiple regression analysis.
Results: 1. Total number of subjects participating in this study is 91 patients and their primary caregivers. Average age of the patients is 63.53 years old. 63.7% were female. Of those 70.3% are illiterate, 38.5% are femoral neck fracture, 50.5% has ORIF, 82.4% has tubes in their bodies while staying in the hospital, 52.7% developed complications. Primary caregivers subjects has the average age of 48.26 years old, 71.4% are female, 68.1% has a middle/high school diploma, 83.5% are Buddhist, 31.9% are the children of the patients, 45.1% believes they have good health, 76.9% does not have similar care giving experience.
2. The score of the need of being excused from the hospital is 11.15 out of the total score of 44, which is 46.4%. The reason for higher scores is the patients have been in ICU and the primary caregiver believes they have average health.
3. The score of the satisfaction level after being released from the hospital for one week is 25.7. The reason for the higher score is because the patient has been to ICU and the primary caregivers are older.
4. The score of the satisfaction level after being released from the hospital for one month is 33.77. The reason for the higher score is due to the patient has been in ICU.
Conclusion: Patients being in ICU and the age of their primary caregivers and their perceived health condition are the primary reasons for the needs of the orthopedic patients and their primary caretaker and their discharge needs satisfaction.
