臺灣博碩士論文加值系統

全民健康保險於1997年3月1日正式實施後，由於採用部份負擔、醫藥分業、診斷關係群等前膽性支付制度之下，造成醫療給付的方式改變，對醫療業產生巨大的衝擊。而近年來隨著醫療科技進步、人口老化以及民眾就醫需求增加，照成了健保費用增加幅度超越健保收入增加幅度。醫院面對著收入大幅縮水情況之下，無法承受虧損的醫院紛紛退出競爭市場，因此醫院數目在近幾年來逐漸減少。醫院考量到無法在價格上進行調整，因此便開始著重於內部成本之控制，一方面降低各方面成本的支出，另一方面則是提升醫療品質服務，方能提伸醫院本身的競爭力。

本研究採用資料包絡分析法(Data Envelopment Analysis﹐DEA)來檢視行政院衛生署「台灣地區公私立醫療院所現況及醫療服務量調查」資料中所評鑑2005至2007年之20家醫學中心經營相對效率之差異性。以此20家醫學中心為研究樣本，並且以專任護理師數、病床數、特定醫療設備數、執業醫事人員數、醫師數做為投入變項，以住院人日、住院人次、手術人次、門診人次、急診人次為產出變相進行分析。分析結果顯示2005年整體有效率之醫學中心有14家無效率的有6家，2006年整體有效率之醫學中心有16家、無效率的有4家，2007年整體有效率的有16家、而無效率之有4家。對於整體無效率之醫學中心，應檢視自身對於投入資源是否適當以及是否有提供優良的醫療服務，並且依照效率參考組合與差額變數分析之結果，對於投入量與產出量做個改善與調整之方向。

In recent years along with medical service advance in technology、aging of population and the people going see a doctor demand increase, causes the nationa health insurance expense to increase range Surmounting health insurance earnings.The hospitals face incomes become shrinkage, can not receive losses and withdrawal the competitive market,therefore the number of the hospital become more and more less.The hospitals think they can not make the adjustment in the price,therefore they become pay great attention the internal cost control. On the one hand reduces various aspects cost disbursement,Another hand promotion medical service quality service.they may promotes the hospital competitive power.

This research uses Data Envelopment Analysis to inspect that from 2005 to 2007 Taiwan area 20 evaluated management of relative efficiency difference the medicine center after the Executive Yuan Department of health. The analysis result demonstrates the majority medicine centers in this three year relatively overall efficiency, the merely technical efficiency, the scale efficiency all good behavior; The few part's medicine center in must strengthen the personnel efficiency with to reduce the cost the disbursement, and strengthens medical service Fang Neng to promote to deliver, promotes the medical potency. Medicine of center regarding the overall inefficiency, should inspect that oneself regarding investment resources to be whether suitable, and result of according to the Slack Variable Analysis, with delivers regarding the investment makes an improvement and the adjustment direction.
