背景：重症末期病人應該坦誠向家屬說明無法治癒之事實，別讓病人遭受臨終前痛苦的凌遲，超過二分之一重症病人有安寧照護需求，在提供重症安寧照護前，首要了解其重要之需求項目，因此引發筆者研究之動機，針對重症安寧療照護需求進行調查，提供更符合重症安寧照護需求的護理服務。

目的：比較加護病房家屬與護理人員對重症安寧照護需求重要程度之看法差異及其影響因素。

方法：本研究為比較相關性研究設計，以立意取樣針對南部某區域教學醫院成人加護病房之家屬及護理人員，於2016年12月20日至2017年4月30日期間收案，自擬「重症安寧照護需求項目調查表」為研究工具，內容有五大需求層面共27個照護需求項目，採用Q方法之問卷調查方式，共收案家屬70份、護理人員70份，有效問卷共140份，並以SPSS 22統計軟體進行建檔及資料分析。

結果：本研究顯示，家屬與護理人員對重症安寧照護需求在心理需求（t=3.82, p=0.00）、靈性需求（t=2.25, p=0.26）及重症需求（t=0.89, p=0.03）有顯著差異。影響家屬對重症安寧照護需求看法之因素有婚姻狀況、與病人同住、其他親人接受安寧照護經驗、家庭同住人數、加護病房天數、疾病嚴重程度（APACHE II）。影響護理人員對重症安寧照護需求看法之因素有婚姻狀況、教育程度、宗教信仰、加護病房工作年資、護理工作總年資、接受安寧照護相關課程。

結論：家屬與護理人員在重症安寧照護需求項目之研究調查，家屬對”危急病況的通知”、”瀕死照護”及”臨終出院衛教”需求之重要程度高於護理人員。因此，改善病危訊息連絡的即時性、建構瀕死照護衛教及臨終出院衛教之數位平台，以優化未來重症安寧照護需求之服務。

Background: Terminally ill patients and their families should be honestly informed by the fact that their conditions are beyond cure to avoid their suffering and pain before death. More than half of the terminal patients need palliative care; however, healthcare professional should understood what palliative care they need before nursing. This is why we conducted a survey of the important needs for palliative care among families and nurses in order to provide nursing care that can better meet the palliative care needs of the terminal patients.

Purpose: This study aims at the comparison of the perspectives between the families and the nurses on the importance of palliative care needs in the intensive care units and its influenced factors.

Methods: This study used a comparative and correlational research design. Purposive sampling was used to recruit samples among families and nurses in the adult intensive care units from a regional teaching hospital of Southern Taiwan during December 20th, 2016 to April 30th, 2017. The research tool was the self-developed “Questionnaire on the Palliative Care Needs”, containing five major needs with a total of 27 items. Q-methodology was used. A total of 140 with 70 families and 70 nurses were retrieved. Statistical software of SPSS22 was used for archiving and data analysis.

Results: The results showed that there exist significant difference between the perspectives of the families and the nurses in terms of psychological needs (t=3.82, p<0.00), spiritual needs (t=2.25, p<0.05) and need for critical care (t=0.89, p<0.05). The influenced factors for the families’ perspectives on palliative care needs for critically ill patients included the marital status, living with the patients, other relatives’ experience receiving palliative care, number of family members living together, number of days staying in intensive care units, and severity of disease (APACHE II). The influenced factors for the nurses’ perspectives on palliative care needs for critically ill patients included marital status, education level, religious beliefs, seniority of nursing work, and having received palliative care related courses.

Conclusion: This survey found that families are more concerned about “informing of critical conditions”, “end-of-life care” and “health education before discharge due to terminal (DDT)” than nurses. Therefore, the improvements in informing the patient’s critical condition immediately and the establishment of a digital platform for the health education of end-of-life care and discharged program should be made to optimize the services to meet the palliative care needs for terminally ill patients in the future.
