摘　要
醫療失效模式與效應分析(Healthcare Failure Mode Effect Analysis,HFMEA)是目前運用於醫療流程的風險評估手法之ㄧ。目前在執行風險評估的過程，發現風險評估準則的嚴重度與發生率仍然無法客觀的給予評分，乃是因為成員的背景與經歷皆有所差異，導致改善程度受到侷限，因此也延長專案執行時間，相對的更增加病人用藥安全風險。因此，為使HFMEA在探討病人安全用藥之風險的結果能夠更貼近實際問題要求，達到提升病人用藥安全的目的。本研究提出整合HFMEA與關聯矩陣的方法進行在急診藥局中病人用藥安全的風險。結合專家認知與實際錯誤數據，透過失效模式、失效原因、用藥等級建立所建立關聯矩陣，來作為風險評分的準則，最後再透過權重來決定改善順序。其中以1分、3分、5分作為關聯係數，分數越大者代表具有高度相關，越小者代表具有低度相關。
結果發現，在藥品部分以處方藥及管制藥錯誤居多，其中最常犯的錯誤是藥名錯誤、劑型錯誤、數量錯誤。另外也發現，導致失效發生的原因是未依三讀五對(Si=1256)、判讀處方失誤(Si=1040)、藥品外觀相似(Si=836)、藥品擺放位置相近(Si=555)、混藥問題不明(Si=540)、與調劑動線相近(Si=422)。以人員、藥品、設備、環境比較傳統HFMEA與本研究所提出的方法結果發現，人員與藥品部分的問題皆相同；設備部份本研究鑑別出新項清潔問題；環境部份則發現調劑動線問題，經實際勘察卻發現有此問題。
本研究所提出的方法確實能以實際錯誤數據來鑑別更多流程中的問題，這可提供醫院一個系統風險評估的手法來幫助醫院更了解問題癥結所在，進而提升病人用藥安全。

Abstract

Healthcare Failure Mode Effect Analysis (HFMEA) is one of the popular methods for medical risk assessment. This method relies largely on evaluator’s subjective judgement to quantify the risk level. The quality of risk assessment and the length of each assessment project, therefore, depend mainly on evaluator’s background and experiences. The result is that many medical risk assessment issues, including the drug safety issue, cannot be effectively managed.
This study integrates HFMEA and Relational Matrix to improve drug usage safety for emergency room pharmacies. The Relational Matrix used in our study are derived from both expert’s opinion and the error data collected from real cases. These error data are represented in the relational matrix through three different measurements: failure mode, failure cause, and drug level. And then, we can give a weight for each failure cause to improve of the ranking. The Relation Coefficient has a value of 1, 3, or 5 in the Relational Matrix. Higher number means that the measurement is highly correlated with the risk factor. Lower number represents smaller correlation.
The results show most drug usage errors are coming from prescription drugs and controlled substances. The main categories of errors include: drug name, drug type (capsule or injection, for example), and prescribed dosage. Comparing our study to other risk assessment studies using HFMEA where various factors including staff factors, medicines types, equipment, and environmental factors, our study identified two new factors: sanitary of the equipment and the prescription filling process. These factors are verified by field visit during our study. Our study can help to improve the operation procedures for emergency room pharmacy. The revised HFMEA method provides a more reliable risk assessment framework. We believe that our study can make hospital aware of problems in prescription drug useage and help to increase drug use safety.
