Comparison of amount of outpatient visits and national health insurance expenditure between Chinese medical practitioners engaged by bureau of national health insurance of Chinese Medical Physician Specialist License Qualifying Exam and Chinese Medicine Physician License Exam in Taichung City in 1999 YU-WEN TSENG Major Professor : KUANG-WEI CHEN Institute of Chinese Medical Science, China Medical College, Taiwan, ROC There are two methods to acquire Chinese medical practitioners (CMPs) license in Taiwan. The first method is through the Chinese Medical Physician Specialist License Qualifying Examination (CMPSLQE). Such examination does not require any medical background. The only requirement is to pass the entrance examination. The second method is through Chinese Medicine Physician License Examination (CMPLE). Such examination is meant for those who received proper Chinese medical education either from Chinese Medical Department (CMD) or from a post-baccalaureate program in Chinese Medicine (PBPCM). However, there are not many studies to compare these two methods. Therefore, based on the 1999 data extracted from national health insurance research database, we have conducted a study that selects the Chinese medical practitioners who practice in Taichung City, a city with the highest concentration of Chinese medical practitioners, and have engaged by the bureau of national health insurance as the subject. First, we started off using t-test method. Then, we perform multiple regression model to adjust these factors, namely sex, age and level of practice place, into account for further analysis to compare the relationship of the amount of outpatient visits and national health insurance expenditure between the Chinese medical practitioners of CMPSLQE and CMPLE. The results of this study as follows: 1) Monthly average amount of outpatient visits: CMPLE is less than CMPSLQE 2) Monthly average proportion of cases: General case: CMPLE is less than CMPSLQE Acupuncture/dislocation case: CMPLE is greater than CMPSLQE Internal medicine case: CMPLE is less than CMPSLQE 3) Monthly average national health insurance apply claims: Generally, CMPLE is greater than CMPSLQE. For claims on acupuncture/dislocation case, CMPLE is greater than CMPSLQE. 4) Monthly average national health insurance total expenditure: CMPLE is greater than CMPSLQE. For claims on acupuncture/dislocation case, CMPLE is greater than CMPSLQE. 5) Age for Chinese medical practitioners: For every incremental of 1 year of age in the Chinese medical practitioners, the number of general case is up by 1% while the acupuncture/dislocation case is down by 0.8%. 6) Sex of Chinese medical practitioners: The number of male doctors on acupuncture/dislocation case is 9.6% more than that of female doctors. 7) Level of medical place: On the chronic diseases case, the hospital doctors who handle such cases are higher than the clinics doctors by 36.6%. The average age of the patients who visits hospital doctors is higher than that of the patient who visits clinics doctors by 5.2 years. The intention of the finding of such study is to let the medical practitioners to gain a better understanding of the Chinese medical insurance situation between the CMPSLQE and CMPLE. Also, it serves as a case study for the health authority in setting their management and Chinese medical policy.
