中文摘要
背景:乳癌是台灣女性發生率最高的癌症,也是國人癌症個案數增加最多的疾病。婦女罹患乳癌的症狀經驗中,以癌因性疲憊最令其感到困擾。故本研究旨在探討乳癌病人癌因性疲憊、中醫體質類型與經絡能量相關性,並進一步探討影響乳癌病人癌因性疲憊相關因素。
方法:本研究採橫斷式研究設計。研究對象為110位高雄某醫學中心腫瘤科、乳房外科病房之乳癌病人。收案期間於2017年11月至2018年1月,以台灣簡明版疲憊量表、中醫體質量表及經絡能量檢測儀為研究工具,測量乳癌病人癌因性疲憊程度、中醫體質類型及經絡能量。並以線性回歸模式來檢測乳癌病人癌因性疲憊預測因子。
結果:乳癌病人中重度疲憊比例高達37.3%,其中以日常工作及生活樂趣影響層面最嚴重。經逐步回歸統計分析結果發現,乳癌病人陽氣虛體質得分越高、罹病時間越長、失眠程度越高,癌因性疲憊程度越嚴重(p<0.01)。正接受化學治療之乳癌病人,疲憊程度顯著高於非化學治療者(p<0.01)。而乳癌病人癌因性疲憊程度則與經絡能量無顯著相關性。
結論:藉由本研究探討和分析乳癌病人癌因性疲憊程度、中醫體質類型及經絡能量的相關性,我們發現陽氣虛體質與癌因性疲憊的假說結果相近,可提供乳癌病人癌因性疲憊中醫藥輔助治療的參考。因此建議在護理實務上結合中醫體質量表評量方法,以提供完整且多元的評估方式。
Abstract
Background:Breast cancer was the leading cause of female cancer in Taiwan, and the most increase in cancer incidence over the past decade. Cancer-related fatigue (CRF) was a common and disabling symptom experienced by patients with breast cancer. The current research was carried out to investigate the association between CRF, traditional Chinese medicine (TCM) constitution and meridian energy, and also assess the confounding factories of CRF in breast cancer patients.
Method: The cross-sectional research was conducted from October 2017 to January 2018. A group of 110 breast cancer patients who had hospitalized in oncology and breast surgery wards at a medical center in the Kaohsiung to receive their standard treatments. They completed the questionnaires of Brief Fatigue Inventory Taiwan Form (BFI-T) and Body Constitution Questionnaire (BCQ), and measured the meridian energy obtained by the meridian energy analysis device (MEAD). Then, linear regression modeling was employed to examine the potential fatigue associated factors in breast cancer patients.
Results: Breast cancer patients who suffered from moderate to severe fatigue (BFI-T score 5-10) was as high as 37.3%. Normal work and enjoyment of life were the most significant interfering items of BFI-T. According to the statistical analysis of stepwise regression, we found that breast cancer patients with higher score of Yang-qi deficiency had longer duration of disease, the higher VAS score of insomnia and higher degree of CRF. Patients have undergone chemotherapy cycle had a higher degree of CRF than whom haven’t. There was no significant correlation between the degree of cancer-related fatigue and the meridian energy in breast cancer patients.
Conclusions: This study aimed to investigate the relationship among degree of CRF, the TCM constitutions, and meridian energy in inpatient patients with breast cancer. Through our study, we found that Yang-qi deficiency constitution and CRF were positively correlated; it could be the reference of TCM syndrome differentiation and treatment for CRF. Therefore, we suggest that combine TCM constitution assessment in current nursing practice to provide the holistic and diverse evaluation.
