臺灣博碩士論文加值系統

本研究目的為探討成人加護病房病人家屬對緩和醫療認知、態度與需求之相關性，採横斷式描述性相關研究，以立意取樣方式選取北部某醫學中心成人加護病房病人家屬共90位，所得資料以SPSS 20.0套裝統計軟體進行分析，研究結果顯示：(1)加護病房病人DNR的簽署代理人50%為兒女。(2)曾與醫師或護理師進行疾病末期緩和醫療家庭會議的家屬佔20%。(3)實際接觸過安寧病人照護的家屬，對於緩和醫療採較「正向積極」的態度，並希望病人能轉入安寧病房接受照顧以達善終。(4)當病人的疾病惡化或對治癒性反應不佳時，醫師第一個應該告知病人本人。(5)成人加護病房家屬對於緩和醫療認知總分為44.32，態度總分為45.22，需求總分為49.07，傾向正向認知、態度及需求重要性。期望本研究結果有助於加護病房醫療人員瞭解緩和醫療對於病人是否能善終之重要性，及早提供家屬相關訊息及病人醫療決策，提升加護病房善終照護品質，減少無效醫療資源浪費。

The purpose of this study was to explore the relationship of cognition, attitude and needs toward palliative care on family members with patients at the adult Intensive Care Unit (ICU). The study was a cross-sectional descriptive research. There were 90 family members, with patients at the adult ICU in a medical center in northern Taiwan, selected by using purposive sampling method. The data were analyzed by SPSS 20.0. The results showed that: (1) Fifty percent of the DNR (Do Not Resuscitate) consents were signed by adult children. (2) Twenty percent of the research subjects had a palliative care family meeting with the physician or the nurse. (3) Family members who had cared holistic patients had more positive attitude toward palliative care, and were more likely to hope that the patient could be transferred to the hospice ward for good end-of-life care. (4) When the patient’s disease deteriorated or the response to the treatment was poor, the physician should inform the patient first. (5) The congnition score toward palliative care was 44.32, the attitude score 45.22 and the needs score 49.07, meaning the positive cognition, attitude and demand importance. It is expected that this study will help ICU medical personnel to understand the importance of palliative care for patients’ good death; provide the early notice and medical decisions to family members; improve care services for patients in end-of-life process at the ICU; and avoid over expenditure of futile medical care.
