Objective: Mandibular molar protraction for complete closure of an edentulous space is a time-consuming procedure and loses alveolar bone. The purpose of this retrospective and preliminary study was to evaluate the effects of platelet rich plasma (PRP) injection on mandibular molar protraction for closing an edentulous space.
Methods: Thirty-seven mandibular molars were protracted to close the edentulous spaces, of which 9 had submucosal PRP-injection (PRP-group) and another 28 had no PRP-injection (non-PRP-group) right before the protraction. The cone beam computer tomography images taken right before (T1) and after (T2) orthodontic treatment were used to analyze the alveolar bone loss, alveolar bone preserving ratio, root resorption, and rate of molar protraction. The intragroup and intergroup comparisons were analyzed statistically by the Wilcoxon singed-rank test and the Mann-Whitney U test respectively (p<0.05).
Results: The rate of mandibular molar protraction in the PRP-group was 1.73 times faster than that in the non-PRP-group. Both of the non-PRP-group and PRP-group were revealed having alveolar bone loss significantly (p<0.05) in almost all aspects of the protracted molars. However, the amount of alveolar bone loss was 36-40% less and the alveolar bone preserving ratio was elevated significantly at the lingual and mesial aspects of the protracted molars in the PRP-group (p<0.05). The root resorption was similar between the non-PRP-group and PRP-group.
Conclusion: Submucosal injection of PRP might accelerate mandibular molar protraction and reduce the alveolar bone loss in mandibular molar protraction for complete closure of an edentulous space
