研究目的與方法：本研究採行動研究，旨在改善安寧療護執行喪親服務的問題，使用焦點解決短期諮商調整電話關懷的服務，而後瞭解實施的情況，並發展適合醫院進行喪親電話關懷的介入策略。
參與者：某醫學中心接受安寧療護的喪親者，共7位。
結論：透過焦點解決電話諮商架構回饋喪親者的悲傷調適方法為可行的介入策略；焦點解決電話諮商架構與電話諮商紀錄表可協助電話關懷的進行，達到引導對話、評估悲傷反應的效果，因而增進執行者的自我效能感；安寧療護團隊與喪親者建立信任關係是進行電話關懷的基礎，執行者對於悲傷輔導的認知基礎可以增進電話關懷的效果。
建議：安寧療護團隊宜落實知情同意以建立喪親者對電話關懷服務的認知，並提供執行者兩日教育訓練以對焦點解決電話諮商有初步的理解，安排定期的督導以提升悲傷輔導的效能。執行者在執行電話關懷時，宜主動引導談話，並保持架構的彈性，關注喪親者的需求，若遇到不熟悉的議題可以依循焦點解決的問句進行討論。

The purpose of this study was to improve the bereavement service from a palliative care unit in a hospital. This study adopted solution-focused brief therapy to telephone care (SFBTT), followed an action research. The participants include one care provider and 7 bereaved people. The author presented the conclusions: SFBTT may develop appropriate intervention strategies for bereavement services in a hospital. SFBTT and SFBTT-record sheets is a tool to orient the conversation and evaluate the grief reactions, therefore, it reinforces the sense of self- efficacy of the provider. The trust relationship between the hospice team and a bereaved person is the foundation of telephone care, and the grief cognition of the provider may enhance the effectiveness of the telephone care. The author suggests that the informed concern may give a bereaved person a well-preparation to the telephone care. The palliative care unit should provide a two-days SFBTT education for both the provider and a scheduled supervisor to maintain the care quality. To the care provider who applied SFBTT, the provider should not only play a positive role in the conversation but also be aware of the flexible structure of SFBTT, and concern about the needs of a bereaved person. If provider encounters the unfamiliar issues, the solution-focused skill might be applied duly in the conversation.
