由於社會環境的進步， 民眾就醫時對醫院的要求也越來越高，再加
上全民健保的實 施， 醫療環境面臨了劇烈的競爭壓力，在新保險給付制
度下保險給付金額固定，利潤空 間縮小的情況下，醫院面臨成本控制的
問題，全面品質管理 (TQM) 在此時被導入醫療服 務業，作為降低成本提
升競爭力的工具。 在新的保險給付制度下病歷是相當重要的給付 依據，
而醫院的病歷管理又面臨了許多問題，所以本論文嘗試將 TQM 與病歷管
理實務做 整合，期望能透過 TQM 的實行而做好病歷管理的工作進而提升
醫院的競爭力。本論文首 先經由相關文獻作探討，並瞭解病歷管理的現
況與未來趨勢，然後將 TQM 與病歷管理特 性作整合分析作為模型建立之
基礎。 在模型建立之初，首先結合病歷管理實務與持續改 進的步驟提出
一個以解決問題為導向的病歷室持續改進的模型，而後參照 TQM 內容與
實 施步驟並整合 TQM 的精神與特色和病歷持續改進模型後，更進一步建
立一個包含了「組 織系統」、「教育訓練」、「病歷管理」和「激勵與
控制」等四項重要元素之 TQM 迴路

The thesis began with the review of related papers and then
managed to study the current status and future trend of the
medical record management. Furthermore, we analyzed the
character of both Total Quality Management(TQM) and medical
record management to aid building a TQM model for the medical
record management. To building the model, we combined the task
of medical record management and the steps of continuous
improvement together to make a problem-solution oriented model
for the medical record department. Moreover an improved TQM
circular model ,including four major items : organization
system, training, medical record management and encouragement
with control, was made and examined by a real case.
