The chronic disease usually can not be cured well but controlling. It can only be controlled by the daily care activity, regular diagnosis and dosage under the physician's instructions. Besides it, doctors must keep the patient's symptom and health care condition as the basis of prescription. It is doubt and unjustifiable that if a doctor's diagnosis for a chronic disease patient by merely measuring few data at present or past. Therefore, if we can provide the home-care measured patient data to doctor's computer for making decision add by communication system, the cooperation relationship between doctors and patients will be increased and valued. Finally, the chronic disease controlling will be getting better from the individual effort. This research tries to find a capable model for monitoring and managing the chronic patients of community by using client/server computer architecture. It is emphasized on the chronic patients of community by collecting their related health care data which were measured by themselves locally. The designed system can accept the measured patient information and store them into the database file server at hospital site by three communication devices. There are three operation models including (1) Measure at home and telephone transmission, (2) Measure via helping and telephone transmission, and (3) Measure at home and computer homepage networking. Running the system, doctors and practicians can take care the chronic patient to improve the health care services and obtain a higher efficiency of curing results. This research is divided into three parts: chronic disease database file server, chronic disease WWW web site service network and chornic disease telephone voice service system. We can use WWW web site service network or telephone voice service system to stoore the measured numerical data from the home care of chronic patient into the database file server. Doctors can get and review their new information from nursing program by internet communication device. Furthermore, we can control patient's symptom condition more correctly and make suitable handling. This research is excepted to cooperate with the relative physicians and nurses of the National Cheng Kung University Hospital to those patients who has high blood pressure disease. From the networking experience, we hope to expand the research model to support the medicine and pharmacy division operation at community care. Furthermore, we will realize and install a prototype model of the community medical network system.
