本研究是以帕斯「人的轉變」護理理論為指引,並採用帕斯現象詮釋法之質性研究。目的在於了解脊髓損傷患者受苦生活經驗的結構,並擴展帕斯之「人的轉變」護理理論。
藉由四位年齡37∼44歲,受傷8∼18年,自願參與研究之脊髓損傷患者與研究者深入對談,述說其生活中與受苦有關的經驗,研究者再將之以帕斯研究法中的「擷取與綜合」和「啟發性的詮釋」做資料分析。得到脊髓損傷患者受苦的生活經驗是:受苦的經驗就是在絕望之中,個人在「渴望支持卻遭摒棄」,「接受協助又不想成為負擔」的矛盾人際關係之中拉拒,因為發現存在的意義,找到自處的平衡點,而寄望、籌劃未來,並激盪出自助助人的力量,能夠看淡傷痛,懷抱希望與自信,開創未來人生。此經驗結構和帕斯護理理論中的「價值」、「連結一分離」、「增強」三個概念有關。而脊髓損傷患者在受苦的經驗中,期望被了解尊重,並能實現人生理想。
此研究結果增進了對脊髓損傷患者生活本質及受苦經驗結構的了解,回應了帕斯護理理論及護理科學的關懷重心,並可應用在護理實務、研究、教育及護理理論之上。
This qualitative research is based on Parse''s theory of "human becoming". Pares''s research methodology was applied in this study. The purpose of this research is to understand the structure of the lived experience of suffering for persons living with spinal cord injury, and to expand the Parsers theory of "human becoming".
Four people aged 37-44 with spinal cord injury for 8∼18 years were invited to participate in this research. They engaged in dialogue with the researcher to describe their experience of suffering. "Extraction-Synthesis" and "Heuristic Interpretation" of Pares''s research methodology were applied to analyze data. The major finding of the study is the structure which is: people were experiencing in a dilemma with others during disappointed situation, because understanding the meaning of being, they could powering themselves and planning the future to achieve their goal. This structure was related to three concepts of Parse''s theory-valuing, connecting-separating, and powering. During the participant''s experience of suffering, they hope to be respected and achieve their goal.
The result of this research improved the understanding toward people with spinal cord injury and suffering experience. It also reflect the focus of Parse''s theory and nursing science and could be applied on nursing practice, nursing research, nursing education and nursing theory.
