本研究著眼於成人油症患者暴露於多氯聯苯以及其代謝產物十六年後的甲狀腺功能的評估。從1994年起在2081個油症患者登錄資料中我們追蹤了154位男性以及244位女性年齡大於32 歲的油症患者並且與113位男性以及268位女性經過社區比對,年齡相近者,比較他們的甲狀腺的功能。
油症患者血液中的甲狀腺賀爾蒙都在正常範圍中,男性油症患者T4有顯著的增加, ( 平均值=7.26 ±1.71 對照於 6.82 ±1.36, 顯著效力= 0.037)。 女性油症患者則有T3降低, (1.99 ±0.09, 2.01 ±0.08, 顯著效力= 0.014 ) Free T3/T3增高(1.82 ±0.43, 1.74 ±0.42, 顯著效力= 0.038)的現象。
油症患者對於PCB的濃度與TSH, FT3, FT3/T3的濃度有相關,其相關係數分別為 -0.136, 0.203, 0.226, 顯著效力分別為 0.031, 0.001, 0.001。再以不同性別來進一步分析,只有男性油症患者的PCB的濃度與FT3, FT3/T3的濃度有相關。其相關係數分別為0.226, 0.240, 顯著效力分別為0.020, 0.007。 相對之下, 女性油症患者則並沒有相關性存在。
本研究的結果顯示, 暴露於多氯聯苯以及其代謝產物十六年後,油症患者的甲狀腺功能的改變是有限的。其作用機轉以及甲狀腺的恆定仍需更進一步研究分析。
The thyroid functions of adult Yu-Cheng patients were studied 16 years after PCBs intoxication. Three hundred and ninty-eight Yu-Cheng persons age over 32 years old were selected and thyroid functions were compared with 381 community-matched subjects since 1994.
The serum thyroid hormones of Yu-Cheng patients were within normal range. Yu-Cheng men had significantly higher serum T4 levels, compared with their controls. ( mean=7.26±1.71 vs 6.82±1.36, P-value = 0.037) The T3 level was lower, (1.99±0.09, 2.01±0.08, P-value = 0.014 ) while the FreeT3/T3 ratio was higher in Yu-Cheng women. (1.82±0.43, 1.74±0.42, P-value = 0.038)
In Yu-Cheng patients, there were correlations between PCB concentration and TSH, FT3, FT3/T3 ratio respectively (Pearson''s correlation = -0.136, 0.203, 0.226, P-value = 0.031, 0.001, 0.001). The relationship is obscure when we further restrict to different sex. Only Yu-Cheng men had good correlation between FT3, FT3/T3 ratio and PCB concentration (Pearson''s correlation = 0.226, 0.240, P-value = 0.020, 0.007 ), while in Yu-Cheng women, there were no correlation between thyroid functions and PCB concentrations. The present results suggest the alterations of adult human thyroid function decades after PCB exposure were limited. The interaction mechanism should be awaited for further studies.
