摘 要
本研究目的主要是了解精神官能症病患照顧者負荷的情形，並探討其相關之因素。本研究正式收取81位精神官能症住院病患主要照顧者為研究對象，主要研究結果如下:
分析精神官能症住院病患照顧者的整體及其四個層面(生理、心理、生活作息、社會互動)的負荷程度發現，全體樣本在整體及其四個次量表的負荷皆屬中度負荷程度，其中又以「心理負荷」最高，「生理負荷」最低。
分析照顧者社會支持需求程度結果顯示:樣本呈現較高的需求，而其三個次量表中，又以實質性支持需求程度最大，其次分別為情緒性及資訊性。而照顧者社會支持需求及其三個次量表皆與照顧者負荷呈統計上顯著正相關。照顧者疾病認知方面與照顧者負荷亦呈顯著正相關，另外生理、心理、社會、家庭互動四個層面與照顧者負荷之相關有意義。疾病表現行為量表中照顧者感受患者求醫及症狀行為困擾程度與照顧者負荷呈顯著正相關。照顧者性格特質方面，以神經質向度與生理負荷呈顯著相關，外向度則與照顧者負荷及其次量表負荷無相關。
患者特質與照顧者負荷間，除負荷次量表的「社會互動」與「宗教」達統計上顯著差異外，負荷整體及其四個次量表皆分別與「疾病表現行為」有顯著相關。照顧者特質與照顧者負荷間，「照顧年數」、「每天處理病患抱怨的時數」、「家庭月收入」與整體及心理負荷達統計上顯著相關。另外，「照顧者疾病認知」與「疾病行為之感受困擾程度」皆與照顧者負荷呈顯著相關。
根據各變項與整體照顧者負荷的關聯情形，共選出五個控制變項(家庭月收入、照顧年數、每天處理病患抱怨時數、照顧者疾病認知、疾病行為之感受困擾程度)，共解釋了照顧者整體負荷44.4%的總變異量。五個控制變項中，又以「照顧者疾病認知」與「疾病行為之感受困擾程度」是負荷的顯著預測變項。調整控制變項的影響後，「照顧者社會支持之需求」又解釋了9.1%的變異量，換言之，此六個預測變項共解釋了53.5%的總變異量。除整個迴歸方程式達統計上顯著意義外，「照顧者社會支持之需求」亦是照顧者整體負荷之顯著預測變項。
生理負荷方面重要的預測變項為「每天處理病患抱怨時數」及「疾病行為之困擾程度」，共解釋生理負荷迴歸31.3%的總變異量。心理負荷方面重要的預測變項為「疾病行為之感受困擾程度」，共解釋心理負荷迴歸21.4%的總變異量。生活作息負荷方面重要的預測變項為疾病認知次量表中的「心理」層面、「疾病行為之感受困擾程度」及「照顧者社會支持之需求」，共解釋生活作息負荷迴歸35.6%的總變異量。社會互動負荷方面重要的預測變項為「每天處理病患抱怨時數」、疾病認知次量表中的「家庭互動」層面及「疾病行為之感受困擾程度」，共解釋社會互動負荷34.5%的總變異量。
本文亦根據研究結果，於護理應用及未來研究提出討論。

Abstract
The purpose of this study was to investigate the burden for caregivers of neurotic disorders patients, and to explore the related factors. Eighty-one caregivers were recruited. The major findings of this study were as follows:
Total burden and its subscales (physical, psychological, daily activity and social interaction burden were to the extent of moderate. The greatest burden was psychological burden; the least was physical burden.
There were significant differences between social interaction burden and religion. Additionally, total burden and its four subscales significantly related with patients'' symptoms. Years of care-giving, hours of processing patients'' complaints and family income were significantly associated with total burden. In terms of caregivers'' characteristics, the more sensitive caregivers were, the heavier their physical burden received. Moreover, caregivers'' cognition of diseases and disturbances of symptoms significantly related with total burden.
In this study, five control variables (family income, years of care-giving, hours of processing patients'' complaints, caregivers'' cognition of disease, and disturbances of symptom) counted for 44.4% variance of caregivers'' total burden (p<.01). When controlling for covatiates, caregivers'' demand for social support increased the variance up to 53.5% (p<.01). Furthermore, the significant predictors for caregivers'' burden were caregivers'' cognition of diseases, disturbances of symptom and caregivers'' demand for social support.
