本研究主要之目的,在探討資訊科技基礎建設所提供之服務與醫院技術效率之間的相關性,並指出最能預測醫院技術效率的資訊科技服務之項目。效率衡量方法以DEA之CCR與BCC二模式為主。投入變數為總病床數、醫師人員數、護理人員數、醫事人員數;產出變數為調整後門急診人次及調整後住院人日。資訊科技基礎建設之八種服務則包括:應用管理、通訊管理、資料管理、安全管理、服務管理、標準管理、資訊科技教育及資訊科技研發等。研究對象為87至89年衛生署評鑑合格之126家地區教學以上醫院,經二階段問卷回收過程後,取得56家醫院之樣本資料,有效回收率為44.44%。研究結果顯示,在醫院技術效率分析方面,醫院的規模與醫院的技術效率,呈顯著正相關性。在IT所提供之服務與醫院技術效率的相關性方面,二者間大多呈現顯著的正向相關性,顯示IT所提供之服務的評估值愈高,則醫院的技術效率亦相對較高。其中,通訊管理及資訊科技教育是同時於CCR及BCC模式中,皆呈現顯著相關性的資訊科技服務,而且以通訊管理最具顯著性。在IT所提供之服務對醫院技術效率的預測能力上,不論是CCR或BCC模式,所得結果皆以通訊管理服務,為最能夠顯著預測與解釋醫院技術效率變動的資訊科技服務項目。
The purpose of this study is to investigate the relationships between information technology (IT) infrastructure services and hospital technical efficiency. CCR and BCC model of DEA were used in this study. The input variables are: beds, number of physicians, nurses, and ancillary medical personnel; the output variables are: “adjusted outpatient and emergency care” and “adjusted inpatient days”. IT infrastructure services include: applications management, communications management, data management, security management, services management, standards management, IT education, and IT R&D. The samples were collected from 126 district teaching hospitals (and above), which were accredited by Department of Health on 1998 to 2000. After two rounds of survey processes, 56 valid hospitals were returned (response rate of 44.44%). The results showed that, there were positive relationships between hospital scale and efficiency score. In addition, most relationships of IT infrastructure services and efficiency score are positive. That is, the higher the IT infrastructure services, the higher the efficiency score were obtained. Communications management and IT education are two services that both significant in CCR and BCC models. And also, the communications management is the most critical one. Either CCR or BCC model, communications management is the service which can most explain and predict efficiency score.
