貝萊嬰兒發展量表第二版(Bayley Scale of Infant Development-Second Edition,簡稱BSID-II)的主要用途在於診斷發展遲緩與擬定治療計畫,由於此量表具有良好的標準化與統計特性,是目前國內使用最好的嬰幼兒發展測驗工具。本研究的目的是在建立BSID-II在發展遲緩兒童的高屏地區常模,並探討不同方式建立之發展指數的適用性、發展遲緩兒童的發展原則與心理計量學特性。參與本研究受試為高雄醫學大學附設中和紀念醫院發展遲緩兒童聯合鑑定中心的591位個案,包含5至43個月以上的嬰幼兒,共分為19個年齡層。
研究結果發現:(1)發展遲緩兒童的心智與動作能力隨年齡而成長,呈直線趨向、二次趨向、三次趨向的發展特徵,性別對心智與動作能力的發展並無影響。(2)本研究所建立的發展指數較外插法與公式(MA/CA ×100)所估計之發展指數更適合來說明發展遲緩兒童的發展現象。(3)發展遲緩兒童符合發展的順序性假設,其發展順序和正常兒童無異。(4)內部一致性係數(a)介於.93∼.99,高於BSID-II指導手冊的數值。(5)測量標準誤(SEM)介於1.50∼3.97,由於本研究的對象是侷限於發展遲緩鑑定中心的樣本,故比BSID-II指導手冊的數值低。(6)與零歲至六歲發展篩檢量表的效標關連效度均達0.01的顯著水準,顯示兩者均是以相同「發展」的概念來編制量表題目。(7)建立各年齡組發展遲緩兒童的高雄地區常模。
本研究所建立的常模對照表適宜描述發展指數低於50的發展遲緩兒童,臨床使用時,建議可利用來了解其發展的優劣勢,進而設計適合的療育計畫與建議,且未來研究更可進一步探討BSID-II評估療育計畫的介入效果。
The instrument of this study was the Bayley Scales of Infant Development-second edition (BSID-II), the primary value of which was in diagnosing developmental delay and planning intervention strategies. The standardization and statistical properties of BSID-II made it one of the best measures of infant development available in Taiwan. The purpose of this study is to establish the local norm of BSID-II for the children with development delay in Kaohsiung, investigate applicability of different method to build index score, the developmental rules for the children with developmental delay, and psychometric properties. Five hundred and ninety-one children, who were refer by pediatrician at Center of Developmental Assessment at Kaohsiung Medical University. They were divided into 19 age groups from 5 to upper 43 month old.
The finding as follows:
(1) The mental and motor abilities of the children with developmental delay were increased along with their ages in a liner trend, second trend and third trend. The gender of subjects weren''t to influence the mental and motor development of the children with developmental delay.
(2) The index score of study is applicability to describe development for the children with developmental delay.
(3) The developmental sequence rules for the children with developmental delay as same as normal children.
(4) The alpha coefficients(a)were between.93 and .99, which were high than data on manual of BSID-II.
(5) Standard Errors of Measurement(SEM)were between 1.50 and 3.97. It was a restricted sample so that these also were lower than the data on manual of BSID-II.
(6) Concurrent validity of BSID-II with SSDD-II is significant. This evidence shows BSID-II and SSDD-II was construct from development concept.
(7) The normative data of 19 age groups from 5 to upper 47-month-old children with developmental delay in Kaohsiung have been established.
The normative data of this study are applicability to describe developmental index below 50 for the children with developmental delay. In clinical, it can to understand strength and weakness of development. Evaluation intervention effect of treatment plan for BSID-II is suggested.
