本研究以紐西蘭白兔VX2肝癌模式,評估硼中子捕獲治療在肝腫瘤的療效。將肝臟帶有腫瘤的白兔分成兩組(每組n = 6),以血管導管經股動脈到達總肝動脈後,給予0.5 ml之B-lipiodol 或0.5 ml之lipiodol作為對照。另外為了評估中子照射對動物體的影響,將肝臟帶有腫瘤的白兔分成三組(每組n=2),分別於腫瘤處直接注射0.2 ml之B-lipiodol、lipiodol與normal saline,24小時後將所有動物送入清華水池式反應器接受超熱中子束照射。結果顯示經肝動脈注射B-lipiodol 七日後,正常組織內的藥物大多已經被排除,但腫瘤組織卻仍留有含硼藥物,其平均值達49.65 ppm,而藥物在此劑量下對動物的正常組織無害。經過中子照射的動物有白血球下降的情形,但仍在正常範圍內,肝、腎功能則無明顯變化,而硼顆粒聚集處並無明顯細胞病變或壞死,且B-lipiodol仍然滯留於小動脈中。評估其腫瘤處與全身的的吸收劑量分別為:12.2、1.4 cGy/min,中子通率則分別為2.196×108、1.273×108 n/cm2/sec,對照先前的實驗的結果顯示,必需提高輻射劑量或組織中的藥物濃度與分佈,才可有效殺死腫瘤細胞。
This study evaluated the effects of boron neutron capture therapy in rabbits using a VX2 hepatoma model. Methods: Rabbits (n=12) carrying VX2 hepatoma were divided into the following two groups: group 1 received 0.5 ml B-lipiodol via hepatic artery, group 2 received 0.5ml lipiodol in the same way. In addition, this study also used rabbits (n=6) carrying VX2 hepatoma for neutron irradiation, they were divided into three groups each received intra-tumoral injection of 0.2 ml of B-lipiodol, lipiodol or normal saline, respectively. The rabbits were irradiated at Tsing-Hwa Open Water Reactor at 24 hours after injection. Results: both group 1 and 2’s normal organ have eliminate most drug after 7 days, but the tumor tissue still contained 49.65 ppm Boron, elevation of aminotransferase were observed, but there are no significant difference between two group. After neutron irradiation, WBC counts decreased slightly in all rabbits but still within normal range. Liver and renal function showed no significant change after therapy and no serious damage in boron-rich tissue. The dosimetric parameters of tumor and whole body were 12.2、1.4 cGy/min,and the neutron flux were 2.196×108、1.273×108 n/cm2/sec. In conclusion, in order to achieve a good therapeutical effect, increase the total neutron flux or the B-lipiodol uptake in hepatoma will be necessary.
