維生素B-6、B-12或葉酸缺乏可能導致血漿同半胱胺酸濃度的上升。許多研究指出高同半胱胺酸血症是罹患心血管疾病的獨立危險因子。相反的,素食者被認為具有較低的高血壓及心血管疾病的發生率。因此本研究目的為比較年輕素食者與非素食者血漿同半胱胺酸之濃度,並探討與血漿維生素B-6、B-12、葉酸之相關性。37位年輕素食者 (男性8位;女性29位) 及32位非素食者 (男性10位;女性22位) 參與本研究,平均年齡分別為 28.9 ± 5.5 及 22.9 ± 1.6 歲。素食者包括全素者3位、奶素18位、奶蛋素16位,平均吃素年限為 5.56 ± 3.64 年。受試者接受體位測量、血液生化值檢驗、血漿同半胱胺酸、磷酸比哆醛 (PLP)、紅血球丙胺酸及天門冬胺酸轉胺酶活性係數 (EALT-AC及EAST-AC)、尿液比哆酸 (4-PA)、血漿葉酸及維生素B-12濃度的測量;另外利用3天飲食紀錄計算營養素攝取量。結果顯示,素食者血漿同半胱胺酸濃度顯著高於非素食者 (13.2 ± 7.9 vs. 9.8 ± 2.2 μmol/L; p < 0.05)。素食者維生素B-6平均攝取量略低於非素食者,但兩組間無顯著性的差異。素食者每日維生素B-12平均攝取量顯著低於非素食者 (0.74 ± 0.67 vs. 3.32 ± 1.99 μg; p < 0.001)。素食者血漿PLP (45.0 ± 23.8 vs. 66.1 ± 27.0 nmol/L; p < 0.001) 及尿液4-PA (3.09 ± 5.37 vs. 3.54 ± 2.39 μmol/d; p < 0.05) 濃度皆顯著低於非素食者。素食者EAST-AC顯著高於非素食者 (2.10 ± 0.53 vs. 1.82 ± 0.28; p < 0.05)。但EALT-AC則在兩組間沒有顯著性的差異。素食者血漿維生素B-12濃度顯著低於非素食者 (260.3 ± 114.5 vs. 421.4 ± 128.0 pg/ml; p < 0.001),但血漿葉酸濃度顯著高於非素食者 (12.57 ± 6.56 vs. 8.66 ± 3.10 ng/ml; p < 0.05)。非素食者血漿同半胱胺酸濃度與飲食中蛋白質攝取量 (r = 0.408, p = 0.0205) 及動物性蛋白質攝取量 (r = 0.384, p = 0.030) 呈正相關。血漿維生素B-12濃度在素食 (r = -0.480, p = 0.004)、非素食 (r = -0.394, p = 0.026)、及pooled組 (r = -0.459, p = 0.000)皆與同半胱胺酸濃度呈負相關。建議素食者應該補充維生素B-12以避免血漿同半胱胺酸濃度上升。
The deficiencies of vitamin B-6, B-12 and folate have been associated with the elevation of plasma homocysteine concentration. Plasma homocysteine has been shown to be an independent risk factor of coronary heart disease. However, it has been reported that vegetarians have lower incidence of coronary heart disease than do nonvegetarians. The purpose of this study was to assess and compare the status of plasma homocysteine and related B-vitamins, and to examine the correlation of plasma homocysteine with plasma vitamin B-6, B-12, and folate in young healthy vegetarians and nonvegetarians. Thirty-seven (8 male; 29 female) vegetarians (28.9 ± 5.5 y) and 32 (10 male; 22 female) nonvegetarians (22.9 ± 1.6 y) completed a 3-d dietary record. Anthropometric and hematological measurements, plasma homocysteine, pyridoxal-5′- phosphate (PLP), urinary 4-pyridoxic acid (4-PA), erythrocyte alanine (EALT-AC) and aspartate transaminase activity coefficient (EAST-AC), plasma folate and vitamin B-12 were measured. Among the vegetarians, 3 were vegan, 18 were lacto-vegetarians, and 16 were lactoovovegetarians. The average history of vegetarian behavior was 5.56 ± 3.64 years. Vegetarian subjects had a significantly higher mean plasma homocysteine concentration than did nonvegetarian subjects (13.2 ± 7.9 vs. 9.8 ± 2.2 μmol/L; p < 0.05). The mean total vitamin B-6 intake of vegetarians was slightly lower than did the nonvegetarians, but there was no significant difference between two groups. Vegetarian subjects consumed mean intake of vitamin B-12 was significantly lower than did nonvegetarians (0.74 ± 0.67 vs. 3.32 ± 1.99 μg; p < 0.001). In the vegetarian group, the concentrations of plasma PLP (58.5 ± 30.9 vs. 85.9 ± 35.2 nmol/L; p < 0.001) and urinary 4-PA (3.09 ± 5.37 vs. 3.54 ± 2.39 μmol/d; p < 0.05) were significantly lower than did the nonvegetarian group. The vegetarian group showed significantly higher EALT-AC value than did the nonvegetarian group (2.10 ± 0.53 vs. 1.82 ± 0.28; p < 0.05). However, there was no significant difference in the value of EAST-AC between two groups. The vegetarians had a significantly higher mean concentration of plasma folate (12.57 ± 6.56 vs. 8.66 ± 3.10 ng/ml; p < 0.05) but a lower mean concentration of vitamin B-12 (260.3 ± 114.5 vs. 421.4 ± 128.0 pg/ml; p < 0.001) when compared with the nonvegetarians. Plasma homocysteine concentration positively correlated with total protein intake (r = 0.408, p = 0.0205) and animal protein intake (r = 0.384, p = 0.030) in the nonvegetarian group. The concentration of plasma homocysteine negatively correlated with plasma vitamin B-12 in the vegetarian (r = -0.480, p = 0.004), nonvegetarian (r = -0.394, p = 0.026), and pooled (r = -0.459, p = 0.000) groups. Vegetarian subjects had lower vitamin B-12 intake and mean plasma vitamin B-12 concentration than did the nonvegetarian subjects which lead to increase plasma homocysteine concentration. Vitamin B-12 might be considered to be a routine supplementation for the vegetarians.
