目的：(1)瞭解高雄市旗津區癌症人口學特徵及對醫療資源利用之情形。(2) 探討癌症患者於診斷確定後，就醫治療行為模式及對醫療資源使用之重要變項。(3) 瞭解癌症不同部位別之特性、分佈、存活情形。(4) 瞭解癌症死亡個案存活月數及住院醫療費用使用情形。
方法：本研究為立意取樣，以高雄市旗津地區居民自87年1月1日至90年6月30日在高雄市九家樣本醫院住院並經診斷為癌症之病患。初級資料以訪問卷作為前瞻性之分析，次級資料則取自中央健保局之申報住院費用清單。利用SPSS 10.0 for window套裝軟體進行描述性及推論性統計分析。
結果：本研究結果發現：(一)65歲以下之癌症病患，對於尋求西醫之外之治療，包括中醫、民俗療法等多種治療，比65歲以上之癌症病患更積極。(二)吸菸、喝酒、與嚼檳榔對癌症病患死亡率有顯著性影響。(三)死亡癌症患者的醫療費用比存活者多。(四)子宮頸癌、乳癌患者，接受手術治療之存活率改善最為明顯。(五)癌症患者選擇手術時較集中在醫學中心。(六)癌症確定診斷時，身體不管是否有明顯症狀(即癌症的期別)，與跨院治療就醫次數並無差異。(七)癌症女性病患與男性病患，使用醫療費用之間，沒有顯著性差異。(八)薪資收入之高低與跨院治療情形，及自付醫療費用與受教育程度高低，兩者雖無統計學上之差異，但這有待更多的數目比較，才能做有意義的分析。
結論及建議：癌症確定診斷後，除病患就醫治療模式對醫療資源利用有差異外，癌症特性、治療方式、預後存活及診斷時身體狀況等亦是影響醫療費用之因素。最後依據本研究結果提出以下建議：(1)強化癌症病患就醫的可近性及方便性。(2) 宣導民眾對癌症的正確觀念，癌症並非絕症，只要早期發現，選擇正確的治療方式均有機會得以治愈。(3)建立醫療資訊溝通管道，積極推動癌症個案管理。(4)建立癌症整合性門住診治療，俾使在減少醫療資源浪費下，以使癌症病患獲得最佳的照護。

Abstract
Objective:
The objectives of this study are to understand the ecology and medical resources of utilization in the cancer patients of the Chi-Ching District of Kaohsiung City, and to investigate the health and illness behavior, and medical resources of cancer patients. The objectives are also to elucidate the characteristic distribution and life span of different cancer patients and to realize medical costs of cancer patients.
Method:
Collect data from cancer patients who were admitted from January 1, 1998 to June 30, 2001 in the nine Chi-Ching District area hospitals as a survey sample. Use the survey information to analyze the statistical data by SPSS 10.0 for Windows method.
Result:
Patients under age 65 had a tendency to seek Chinese medical treatment and ancient cultural medical care in addition to modern, Western medical treatment. Cigarette smoking, alcohol consumption, and betel-nut chewing have a statistical contribution to the increased death rate among the cancer patients. Medical costs for survivors were much higher than medical costs for the deceased during this survey period. Surgical intervention improved those suffering from cervical carcinoma and breast cancers. It was found that most cancer patients were treated in the medical centers. Once the diagnosis was made, there was no correlation between the seeking of different hospitals for medical treatment, and the severity of the symptoms of the patient. There was no statistical difference in medical costs between male and female patients. Education level and family income were not factors for cancer, however, an additional number of patients may need to be surveyed for an objective analysis.
Conclusions
Based on this investigation, different types of cancer may affect the course of treatment. It is recommended that: Cancer patients should have nearby and conveniently located facilities for treatment. The public should be advised that cancer is not incurable. Early detection and adequate treatment always to provide best prognosis. The establishment of a sound avenue for medical information and a solid medical record of cancer cases is very much encouraged. The establishment of hospitalization combined with a patient clinic may reduce medical costs and benefit the recovery of the patient.
