本研究主要目的在於瞭解腦中風病人或其家屬選擇長期照護方式的分佈情形，對於腦中風患者在社會人口學特質、家庭照護資源、健康及照護需要等方面的差異，並探討影響腦中風病人或其家屬選擇長期照護方式的因素，同時藉由選擇長期照護方式相關預測因子的了解，使得醫院在出院準備服務時可以幫助出院病人儘早得到適合的照護。進而提供有用的指標，以做為衛政、社政單位、健保局、長期照護機構在資源分配、需求評估及決策重點的參考依據。
本研究將長期照護方式分為非正式（居家）照護與正式（機構）照護二類，以Andersen(1995)醫療利用行為模式為理論基礎，並參考國內外關於「使用長期照護之影響因素」，自行設計之問卷收集初級資料，並經由專家的審查與修正，以達內容的可靠與正確。採橫斷性研究方式，於2002年2月22日至4月22日期間，由高屏地區二家醫學中心之資深護理人員，針對該院腦中風住院病人或其家屬，於出院前三天左右進行面訪收得之腦中風住院病人共347位。
研究結果發現86.2%選擇由家人居家照顧，7.8%選擇機構照護，6.1%選擇請看護（或外勞）在家照顧，主要的第一考量因素是病情程度，佔48.4%，第二考量因素則為家庭中有人照顧，佔46.7%。病患之年齡、意識狀況、日常活動功能嚴重度、特殊醫療照護及輔助性器材使用情形、住院期間有否請看護、居住情形，及主要照顧者之教育程度與病患的關係、自覺負荷情形、家庭成員數等與選擇照護方式有相關。利用對數迴歸分析發現日常活動功能嚴重度、住院期間有否請看護、城鄉居住別、主要照顧者與病患的關係及家庭成員數是選擇長期照護方式的重要影響因素。
由於研究顯示目前腦中風病人出院後仍然是由家庭承擔大部分的照護責任，故本研究建議醫院因加強出院準備服務，提供家屬適切的資訊，以提高出院病人對後續照護的可近性，建立完善的轉介機制，政府及長期照護機構更應發展長期照護替代方案及充實社區中的居家支持服務，以落實連續性照顧之理念。

OBJECTIVE：The purpose of this research was to investigate the various choices that stroke patients and their families have when considering long-term care of the patients. We analyzed the differences and similarities of choices within social demography,the resourcesrequired for home care, and various health care needs. As a result , we were able to predict what factors impact the long-term care decisions of the stroke patients and their families. The research helped ensure that patients receive appropriate care as early as possible by informing the family of the crucial issues they might face when thepatient is released from the hospital. The Institute of Health Policy, Social Policy, the Health Insurance Bureau, and the Long-Term Care Institutes will use this research as references in resource allocation, needs assessment, and policy definitions.
METHODS：This research divided long-term care into two kinds: Informal care by family and formal care in an organization. We based our research on Anderson’s theory(1995), a model of health service usage. That is , long -term care is determined by patients’ behavior patterns. We considered the influential factors of using long-term care domestically and abroad to design the questionnaire for this study. The questionnaire was evaluated for good reliability and validity. Based on cross-section research, from February 22, 2002 to April 22, 2002, using the questionnaire, senior nurses in two hospitals in Pingtung and Kaohsiung interviewed stroke patients and their families threedays before they left thehospitals. At the end, we interviewed 347 patients in total.
RESULTS：The results showed that 82.6 % of the patients interviewed chose to be cared by their family; 7.8 % chose to be cared in an organization; and 6.1 % chose to be looked after by nursing persons (e.g., foreign laborers) at their home. The first factor they considered was their degree of sickness, which accounts for 48.4% of the variance. The second factor they considered was if any one in the family could take care of them, which accounts for 46.8 % of the variance. This is also related to patients’ age, consciousness level, and degree of activities of daily living on their own . Other factors were: Special medical care they have, the medical supplies they use, nursing persons they hired during their hospital staying, their living at home, and the relationship of the patients and the caregivers. After logistic regression analyses, we found that the important factors that influence long-term care choices were: Patients’ activities of daily living, where they live, the relationships between the patients and the care givers, and the nursing persons they hired, and the availabilities of their family members.
CONCLUSION：This study showed that nowadays the families still took most of the responsibilities to look after the stroke patients after they left the hospitals. Therefore, we suggested all hospitals have to strengthen the services for patients and to provide the suitable information the patients’ families need. This is to ensure the patients get continuous care after they left the hospitals and to build a good transfer system betweenhospitals. It is imperative that the government and long-term care organizations develop substitute cares for long-term care and improve the resources available to stroke patients
and their family. It is very crucial to fulfill the ideas of continuous care giving to benefit many people in the society.
