對於新生兒能量的供給一值都是藉由醫護人員的經驗法則來做主觀的判斷,然而每個新生兒的能量消耗都不盡相同,所以不能以括蓋全的對每個新生兒都提供相同的能量供給,而為得知每個新生兒的能量消耗和新陳代謝的速率,我們研製一能量消耗系統來監測、分析,以提供醫護人員做客觀的判斷。
所使用的方法為量測氧氣消耗量和二氧化碳產生量的間接熱量法,此種方法可量測人們因氧化食物所消耗的能量而估算出能量消耗和代謝速率。我們所用的間接熱量法是利用流量穿過法(flow-through technique)來量測經由呼吸反應後氧氣的消耗量和二氧化碳的產生量,由於是非侵入式的特點,所以已廣泛的使用在能量消耗監測的系統上。
量測系統是用一電腦下控制參數給單晶片微控制器做各裝置控制,為一自動化的監測系統。各方面的改良較之前的量測系統更具有簡易的操作介面、硬體設備也縮小、更適合實際的臨床應用。
The amount of energy supply for neonates is always based by doctor’s experience to do subjective decision, but each neonate has different energy expenditure so that it can not provide the same amount energy to each one. In order to know energy expenditure and metabolic rate of every neonate, we design an indirect calorimeter system to monitor and analyze the energy consumption that can help doctor or nurse to do objective decision.
The method of measurement is so called indirect calorimetry that measu- res oxygen consumption and carbon dioxide production. It can estimate energy expenditure and metabolic rate cause of foods oxidation. The indirect calorimetry is based on an open flow-through technique measuring oxygen consumption and carbon dioxide production due to respiration of a neonate. Because flow-through technique is a noninvasive measurement that is widely used in energy expenditure monitor system.
The implemented system uses a computer to transmit control parameter to single chip microcontroller which controls every device. In all respects, the improvement of a measure system is better than previous one and it has easy operation interface, reduced size of hardware, and adapted realistic pediatric clinical application.
