相關文獻多有論述:當突然被告知孩子患有先天性心臟病時,母親除了產生震驚、不確定和焦慮的感受,甚至會出現不知所措的情況而影響對孩子的照顧,故本研究目的即在診斷期以系統性護理指導提供罹患非發紺型先天性心臟病童母親,並探討其不確定感的改善情況。
  本研究採類實驗法中之前後測設計,經立意取樣選擇60位初次診斷為非發紺型先天性心臟病童母親為研究對象,皆來自北部地區一所醫學中心、三所區域教學醫院、兩所地區教學醫院、及一所健保局門診中心的小兒心臟科門診。其中以接受系統性護理指導之30位母親為實驗組,30位接受常規指導者為控制組。控制組係在其子女就診前予以前測,病童就診並接受診斷性檢查,醫師確立診斷並解釋檢查結果後,給予常規指導後控制組立即予以後測;而實驗組則於常規指導後加給系統性護理指導,再施予後測。
  本研究使用之工具分為「病童及母親基本資料調查表」、「雙親不確定感量表」及「系統性護理指導手冊」三部份。藉由訪談病童母親及填寫問卷方式收集資料。所得資料以SPSS 9.0套裝軟體進行次數分配、平均值、標準差、百分比、卡方檢定、t檢定、配對t檢定及單因子變異數分析等進行統計分析,以了解系統性護理指導對非發紺型先天性心臟病童母親不確定感之影響。
  研究結果發現:控制組在常規指導後,病童母親不確定感呈有意義的降低(p<0.01),實驗組在系統性護理指導介入後不僅其不確定感顯著降低(p<0.01)外,其減低情形明顯大於控制組(p<0.001),尤以不確定感分項中的模糊性降低情形更具統計上的意義(p<0.001)。由此顯示因為系統性護理指導提供了有關疾病的相關訊息,使得母親不確定感減輕較多,且對不確定感的第一因素模糊性-意指有關執行計劃及照顧病童的訊息缺乏或含糊等方面,最可降低母親的不確定感。
  系統性護理指導能提供母親其子女疾病的症狀、診斷、治療、照顧、就醫場所等相關資訊。當母親具備照顧病童的常識時,將可協助母親提昇對疾病的認知一致性及照顧能力,進而減輕母親不確定感與改善對其患病的子女適應和照護。
