多項研究證實,非預期性入兒科加護病房對病童母親而言,不僅是一個高壓力高焦慮的情境,且確實需要各方面不同的支持與協助。有鑑於此,本研究先著手研擬一套系統性護理指導並實施之,進而探討其對非預期性入兒科加護病房病童母親焦慮降低程度之影響。
本研究方法採類實驗法(Quasi- experimental design)中之前後測量設計(pretest- posttest design),於民國90年4月至10月間在北部兩家醫學中心的小兒科加護病房,採便利取樣(convenience sampling)方式訪談60位2-6歲非預期入兒科加護病房病童母親,以各30位控制組和實驗組之區隔,分別接受一般性護理指導及系統性護理指導。研究過程中,母親填寫「病童與母親基本資料表」和「焦慮程度量表」等問卷,藉以收集個案之基本資料和評量護理指導前後母親焦慮程度的情形。資料經由描述統計和推論統計分析。
研究結果顯示:(1)﹒非預期性入兒科加護病房病童母親焦慮程度受到孩童年齡、排行和母親教育程度之影響。(2)﹒控制組的母親在接受一般性護理指導後,「環境」(p<0.001)、「情緒」(p<0.001)和「照顧」(p=0.011)對其造成的焦慮程度呈現顯著降低(p<0.05)的情形。(3)﹒實驗組的母親在接受系統性護理指導後,除在「環境」(p<0.001)、「情緒」(p<0.001)及「照顧」(p<0.001)之外,「症狀」(p<0.001)的焦慮程度亦呈現顯著降低的現象。另外,(4)﹒經推論統計分析發現,實驗組的焦慮降低程度明顯大於控制組(p<0.001)。
綜合上述研究結果,或可證明臨床上實施系統性護理指導,較能提供非預期性入兒科加護病房病童母親確實的資訊,同時較一般性護理指導有效的降低母親的焦慮程度。因此本研究建議落實系統性護理指導應可作為提昇護理教育及臨床重症護理品質的參考。
