臺灣博碩士論文加值系統

本研究係採茁根理論﹙ground theory﹚研究法。研究目的為建立失智症老人的主要照顧者參與照顧過程中自我充能過程的概念性理論。以半結構式訪談指引進行深入訪談，理論性取樣共九位居住於北部的主要照顧者參與研究。訪談過程以錄音紀錄隨後逐字轉譯為文字稿，並以持續比較分析的原則進行資料的開放性編碼、主軸編碼和選擇性編碼等程序直至理論呈現。
結果發現失智老人的主要照顧者其自我充能過程共包含六個要素，分別是「孝道義務感」「無助失控感」「內在意識」「照顧能力」「心靈重整」和「生活的經營」。「孝道義務感」和「無助失控感」二者為主要照顧者充能過程的先驅類組，激發照顧者的「內在意識」。內在意識具有如渦輪般的動力除了啟動充能過程，更在不同階段間幫助照顧者發揮審視自己處境、自我省思和權衡的功能，使照顧者「照顧能力」、「心靈重整」和「生活的經營」三部分能獲致發展與成長。
「照顧能力」、「心靈重整」和「生活的經營」此三部分由淺入深各自包含三階段，透過時間的催化，照顧者在「照顧能力」上可由「做中學」、「建立互動典範」到「建構經驗」；「心靈重整」則自「情緒調適」、「情感凝聚」到「情緒智慧」；「生活的經營」的部分從「定位」開始，到「勝任自己」繼而「積極共享」；以平行層面來看，此三部分的各階段彼此間又是平行並有互為助力或阻力的關係。
充能過程也受著「助力」與「阻力」的影響，此二者如同一體兩面其間並無明顯界線，端賴主要照顧者的面對與運用，只要助力大於阻力，充能過程便能持續。整個充能過程蘊含著「力量」的流動，這些力量具有彼此競爭、相輔相成的特性，最初可能需要靠外在因素激活，但只要被激活便會引發照顧者意識上、行動上、態度上及看法上的改變與增長，亦即隨照顧過程的進行，主要照顧者會漸漸發展出自己的「力量」，而這些力量在增長成熟之餘仍為照顧者所用，成為其源源不絕的動力。
此概念架構可提供護理人員作為指引，對失智症老人的主要照顧者各階段心理轉變更有感知；此外亦可引導主要照顧者覺察到自己在「照顧能力」、「心靈重整」和「生活的經營」等部份的成長增進，對參與照顧過程產生正向感受。本研究是國內首篇以失智症患者的主要照顧者為對象所進行的自我充能過程探討，所發現的概念性知識將可供未來進一步實質理論發展的參考。

ABSTRACT
The purpose of this qualitative research was to explore and develop the process of self-empowerment in primary caregiver of dementia elderly. Nine primary caregivers who lived in northern Taiwan were included by theoretical sampling procedure. In-depth face-to face interviews were tape-recorded and transcribed verbatim. Those data were analyzed by ground theory method.
This research presented the development model of self-empowerment process experienced by primary caregivers who were involved dementia elderly caregiving process. Six main components were emerged in this model: filial piety, feeling of out of control and helplessness, inner awareness, care ability, mental reconstruction, and life management. This model starts from the senses of 〝filial piety〞,〝feeling of out of control〞, then evoked caregiver’s 〝inner awareness〞. The inner awareness of caregivers powerfully influences their 〝care ability〞, 〝mental reconstruction〞and 〝life management〞, which are the three parts of inner mechanism, in the caregiving experience. Each parts has three stages which are all permeated by the 〝inner awareness〞. The part of 〝care ability〞includes those stages of trial and error, interacting paradigm establishment, and experience-rule forming. The part of 〝mental reconstruction〞has emotional adjustment, family cohesion, and emotional maturity. The part of 〝life management〞includes those stages of role reposition, capability and confidence increase, and sharing actively. Every lower stage in every part supports the higher stages. The same stage of three parts is paralleled with each other; in addition influence each other by providing supports or barriers. The facilitating factors and barriers from the outer environment provide crucial influences to this complete model.
The results of this research can increase the knowledge in understanding the process of the psychology and self-empowerment in the primary caregiver of dementia elderly. The findings can also provide important information for developing guides for primary caregiver who take care dementia elderly that will facilitate caregiver’s positive feeling in caring involvement.
