臺灣博碩士論文加值系統

自1995年台灣地區施行全民健保以來，醫療費用快速成長，故醫療費用的控制成了主要議題。在全民健保改革方案中提到的論人計酬普遍施行於美國、英國及荷蘭等先進國家，卻發生如風險偏好選擇及刮脂的現象。本文先以2000年台灣地區家庭收支調查以健保分局所轄地區為單位，以家庭為單位分析各區醫療資源的使用情況及認定其影響因子，並藉由拍賣理論及代理理論，本文建立論人計酬下之支付模型，分別推演當一個地區有愈來愈多醫療院所時，醫武競爭的結果及醫療資源分配的原則，其研究結果如下：
1.在健保六分局中發現各區之間醫療資源短絀情形不一，尤其東區及南區各家庭接受健保給付額減去健保保費額高達50﹪以上的戶數為負值，而台北區及北區則不滿40﹪，顯示各區之間醫療資源互相支援的現象。
2.在論人計酬下，同一地區醫療院所數過多會發生醫療服務量下降的情形，可以行政審查及法律救濟的方式加以解決。
3.在醫療資源固定的情況下，除了根據各科投入成本分配外，尚須對風險高的科別增加風險貼水，可加強對醫療消費者權益的保護。

The medical expenditure has been increased rapidly since the National Health Insurance (NHI) in Taiwan was established in 1995. It is important to design a better payment scheme to control the ever-increasing medical expenditure. Since the capitation scheme has broadly been implemented in many developing countries such as USA, England and Nederland, it was also proposed in NHI reform policy in Taiwan recently. However, capitation scheme was often been commended with the drawback on preferred-risk selection and cream skimming. This study utilized the data reported in “the Survey of Family Income and Expenditure in Taiwan in 2000” to analyze the use of medical resource base on the family unit in six regional branches of the National Health Insurance in Taiwan. This study also identified the factors that influenced the magnitude of the payment minus the cost of insurance of each household. Based on sealed-bid model and principal-agent theory, this study built the pay-off models under capitation to specify the equilibrium of medical arms race when there are more and more hospitals in a region and the principle of medical resource distribution within various health care departments. Followings are major findings of the study:
1.The magnitude of the payment minus the cost of insurance under family unit is very different by the region set by the Branch Bureau of the National Health Insurance. Over 50% of the households in the East and South regions experienced negative sign. However, only 40﹪ of the households in Taipei and North regions shows a negative sign. It means that medical resource is supported among them.
2.Under capitation, medical service would decrease with the increasing of hospitals in a region. The problem might be solved through some proper administrative and lawful procedure.
3.Given the medical resource is fixed, risk-premium should be considered in the distribution of medical resource among different-risked health care departments to improve the benefit of patients.
