臺灣博碩士論文加值系統

政府為積極照顧國民健康，於1995年開辦全民健康保險，而健保局為控制醫療費用的逐步上漲，陸續提出論病例計酬制、論人計酬制及總額預算等政策來因應；同時民眾的就醫可近性因保險普及而大幅提高，醫療服務市場之競爭性亦相形提昇，醫院因應市場變化而有整合之議。
本研究先以醫院間合作型態進行文獻探討，並深入瞭解醫院整合模式，及台灣醫療產業的現況和未來發展趨勢；同時亦探討現今醫院整合需考量的因素。分析內容即以現金流量折現法試算台北某市立醫院之經濟價值。分析結果指出，醫院的價值，除針對其經濟價值外，另應考量其服務、教學及研究之成果，同時亦應考慮其整合對象、整合方法、整合時間和地域問題等因素。以本案為例，與同質性公立醫院進行整合，成功率較高，但亦應注意整合後妥善處置兩院事宜。

Taiwan government implemented National Health Insurance program in 1995 to meet the medical needs of its citizens and BNHI has introduced various payment policies to mitigate the growth of the medical care expenditures. Also, the accessibility to health care is substantially enhanced due to the expansion of insurance coverage. Consequently, the competition in the market place is intensified and the issues of hospital consolidation ensue.
This study first performs a thorough literature review on the types of hospital collaboration and hospital consolidation model and the development of the hospital industry in Taiwan. It also covers the factors which may impact on the consolidation of hospitals. Analyses were conducted through the application of discounted free cash flow method to assess the economic value of a municipal hospital. The research results indicated that the value of the hospital considered for consolidation, other than its economic value, should also include its performance in service, teaching and research. In addition, the consolidation target hospital, consolidation model, timing and geographic factors will also attribute to the success of the consolidation. In the case of the municipal hospitals, it will achieve a higher success rate if the consolidation is between hospitals of similar attributes. Attention is also called for the hospitals involved after consolidation is completed.
