本研究的目的利用聲學分析的方式,幫助了解遠距鏡檢查時的發音機轉,與正常狀態下的差異,以作為喉頭遠距鏡檢查判讀以及選擇提示語音的參考。
本研究以30名嗓音評估正常男性為研究對象,利用聲學分析方式,比較在正常發聲狀態下、口咽麻醉後遠距鏡操作前、與喉頭遠距鏡操作時,提示發/ㄧ/ 音與發 /ㄝ/ 音之發聲基礎頻率、頻率變動率、振幅變動率、諧音噪音比、及母音F1、F2共振峰頻率值特性,以評估喉頭遠距鏡檢查對發聲行為的聲學影響。結果顯示,遠距鏡操作中的嗓音,比正常狀態下發聲與口咽麻醉後發聲時的嗓音,有較高的基礎頻率,及較差的頻率變動率、振幅變動率、及諧音噪音比,其結果均具有顯著差異。遠距鏡操作中的母音,比正常狀態下發聲與口咽麻醉後發聲的母音,有較高的F1共振峰頻率值,及較低的F2共振峰值,其結果均具顯著差異。本研究結論為,遠距鏡操作會改變正常男性的嗓音與母音特性。遠距鏡操作時的聲學變異來自於操作的方式,因此臨床判讀時必須考慮是正常的變異還是真正的異常。遠距鏡操作應同時運用聲學評估,以增加遠距鏡結果判讀的可靠性及信賴度。遠距鏡操作時提示音建議為 /ㄝ/ 音。

The purpose of this study is to investigate the speech physiology in telescopic examination from acoustic analysis to provide diagnostic information and suggested phonation in the examination.
Thirty males with normal voice were included in this study. The subjects were asked the subjects to phonate “i” and ”oe” in nature situation, local anesthesia of oral cavity, and during telescopic examination. Acoustic analysis of fundamental frequency, jitter, shimmer, harmonic to noise ratio, and F1 & F2 formant frequency for vowel of “i” and ”oe” in the three different conditions were executed.
The statistic analysis revealed that voice characteristics of fundamental frequency of both vowels in telescopic examination are significantly higher than those in normal situation and local anesthesia. Jitter, shimmer, and harmonic to noise ratio are significantly worse in telescopy than in the other two situations. For either vowels, the F1 formant frequency is significantly higher and that of F2 formant frequency is significantly lower in telescopic examination than those in normal situation and local anesthesia
The study shows the manipulation of telescopic examination changes acoustic characteristics of voice and vowels. When doing interpretation, laryngeal variations should be ruled out from normal variation in manipulation. It is suggested to use the phonation of /oe/ in telescopic examination. The acoustic analysis of phonation along with telescopy can help to improve the diagnostic reliability and accuracy.
