Sulbogin 為一個已經申請專利的新劑型傷口癒合用藥,由動物實驗及臨床試驗均發現,經 Sulbogin 處理過之傷口,具有明顯促進傷口癒合之效果。 Sulbogin 之主成分為兩種目前在臨床上已分別使用的藥物,分別為次沒食子酸鉍 (bismuth subgallate) 及龍腦 (borneol)。 Bismuth subgallate 可活化凝血因子XII,具止血、減少胃液分泌、抗微生物、降低抗 Cholera toxin 抗體產生、降低巨噬細胞分泌一氧化氮等功能。 Borneol 則為 microsomal enzyme 之抑制劑,具抗黴菌作用,在中藥上認為具有消炎、消腫、生肌、鎮定之功能。
傷口癒合為一個相當複雜且調控嚴謹的過程,需要很多分化細胞及組織的共同合作,正常的傷口癒合可粗略分為四個階段:凝血反應、發炎反應、細胞增生和組織重建。在發炎反應階段,巨噬細胞會產生大量的前列腺素E2 (PGE2),前列腺素E2是發炎反應的主要介質,會引起疼痛、發熱、紅腫等不適症狀,因此降低前列腺素E2生成可減緩發炎產生的不適症狀,並且減少對正常組織的傷害。此外,前列腺素E2會調節巨噬細胞分泌一氧化氮,而一氧化氮也具有調節前列腺素E2生成的能力。本實驗目的是要探討 Sulbogin對於發炎階段巨噬細胞合成前列腺素E2 能力之影響,以了解Sulbogin是否有抑制發炎反應,促進傷口癒合的機制。
在本實驗中我們發現僅 bismuth subgallate 會減少lipopolysaccharide (LPS) 活化之 RAW 264.7巨噬細胞株 PGE2 生成量。我們以西方墨漬法分析 cPLA2 蛋白質的表現量,以及測量花生四烯酸的釋放;結果顯示 bismuth subgallate 對於以 LPS 活化之巨噬細胞株 PGE2 生成的抑制,並非經由抑制 cPLA2 或是抑制花生四烯酸的釋放。再以西方墨漬法分析 COX-2 和 mPGES 蛋白質的表現量,發現隨著 bismuth subgallate濃度增加, COX-2 的表現量隨之上升,相對的 mPGES 的表現量隨之下降。進一步測定 COX-2 的peroxidase酵素活性,隨著 bismuth subgallate 濃度增加,COX-2 peroxidase 酵素活性隨之遞減。給予外加的 NO donor-SNP,發現可以抑制 bismuth subgallate 對於 RAW 264.7巨噬細胞株生成 PGE2 的影響。
綜上所述, Sulbogin 中的 bismuth subgallate 會抑制巨噬細胞株生成 PGE2 。 Bismuth subgallate 抑制巨噬細胞株生成 PGE2 可能是經由抑制 COX-2 這個酵素的 peroxidase 活性和 mPGES 的表現;但此種情況可以經由外加 NO donor-SNP 回復。且在RAW 264.7巨噬細胞中對於 PGE2 的生成可能有一個負迴饋的調控機制。因此Sulbogin 可能可以抑制發炎反應,進而促進傷口癒合。
