本研究係以質性研究深入探討高齡初產婦在產後初期執行新生兒照護活動之自我期許行為。研究對象共七位年齡在四十歲以上的高齡初產婦。研究方法是採以低結構性訪談指引進行深度訪談的方式收集資料。研究者在研究對象生產後於住院期間開始進行訪談，並於研究對象返家後連續第一、二週進行家庭訪視，每位研究對象共計有三次的訪談記錄，每次訪談時間約為40-60分鐘。資料收集後，以電腦打字將錄音之內容轉錄為敘述體（narrative）的型式作成行為過程實錄，並將訪談資料以內容分析法有系統的將資料加以分析、歸納、數碼化後，共得1589個行為單元。
研究結果發現，高齡初產婦的自我期許行為可歸納成三大類別，分別為期許之建構行為、評價行為及調整行為。其中以期許調整行為的發生率佔最高而高齡初產婦所執行的新生兒照護活動，共包括有滿足新生兒的生理需求、建立親子（38.52%），其次為期許建構行為（34.86%），而期許評價行為為最低（26.62%）。關係以及提供保護三大部分。其中以滿足生理需求佔最高（49.4%），其次為提供保護（38.45 %），最低為建立親子關係（12.15 %）。
此外，產後隨時間的進展，高齡初產婦的三種自我期許行為皆於返家第一週急劇升高，期許建構行為與期許調整行為於返家第二週仍維持與第一週相近的發生率，而期許評價行為則於返家第二週明顯下降。至於高齡初產婦所執行的新生兒照護活動中，主要為滿足新生兒之生理需求為主，在滿足生理需求與提供保護方面，皆於返家第一週呈現巨幅的上漲，之後於返家第二週之時，滿足生理需求明顯下降，而提供保護仍維持與返家第一週之時有近相同的發生率，唯建立親子關係之部份，在產後初期是呈現緩慢上升的趨勢。本研究之結果，可供護理人員了解高齡初產婦之需求以提供適切性護理照護，進而提升產婦對護理照護服務的滿意度。

The purpose of this qualitative study was to explore the self-expectancy behavior in performing newborn care of primipara with age over 40 years old during early postpartum. The research subjects included seven women who were first-time mothers after aged forty. This study was conducted with in-depth interview by the semi-structure method. Data collection started from hospitalization after childbirth to 1st and 2nd.week after the subject was discharged. Each subject was interviewed three times, each lasting 40-60 minutes. After each interview, the researcher generated narrative process recording expressed by those subjects. By means of content analysis ,the verbal content of the subject were systematically analyzed and categorized, ie.1589 behavior units were inductively coded.
The results indicated that these subjects basically have three concept of self-expectancy behaviors：constructive behavior, evaluating behavior, and adjustive behavior. Adjustive behavior had the highest percentage（38.52%）, followed by constructive behavior（34.86﹪）and evaluating behavior（26.62%）. On the other hand, the contents of performing newborn care included three activities：satisfying physical needs, establishing parent-infant relationship, and providing protection for her infant. Satisfying physical needs had the highest percentage（49.4%）, followed by providing protection for her infant（38.45%）and establishing parent-infant relationship（12.15%）.
Self-expectancy behavior and performing newborn care of these subjects were varied at early postpartum periods. The results of this study serve as a guideline for nurse to further understand the needs of primipara with age over 40 years old and lead to better nursing practice.
