目的:探討地震後半年及三年倖存者罹患創傷後壓力症候群(posttraumatic stress disorder, PTSD)與其生活品質的關係。
方法:此為社區性之追蹤研究。在九二一地震後半年及三年,在魚池鄉,由受訓過之訪視員完成基本資料與「災難相關精神症狀篩檢表(Disaster-Related Psychological Screening Test, DRPST)、簡易生活品質量表-36 (Medical Outcomes Study Short From-36, MOS SF-36)」評估,訪視十六歲以上居民,共計完成1756人。並依二個時間點是否罹患創傷後壓力症候群而將居民分成持續疾病組、復原組、延遲發病組及持續健康組等四組,進行生活品質的比較,所有資料均以SPSS 10.0中文版的統計軟體來分析。
結果:以災難相關精神症狀篩檢表(DRPST)得分3分(含)以上者,診斷為PTSD的盛行率,在地震後半年為23.8%,地震後三年降到4.4%。不管在震後半年或三年,「MOS SF-36」的八大層面上,均顯示罹病之居民生活品質較差。此外,隨著病程的改變,PTSD患者三年後復原者生活品質進步最多,其次依序為持續健康組、持續疾病組,而延遲發病組者生活品質惡化。
結論:居民罹患PTSD的比例在震後三年會明顯降低,而是否得PTSD會明顯地影響震後倖存者的生活品質。
Abstract
The Association of Post-traumatic Stress Disorder (PTSD) and Quality of Life among Earthquake Survivors in Taiwan
Objective:
The aim of this study was to prospectively evaluate the relationship between the clinical course of post-traumatic stress disorder (PTSD) and quality of life among earthquake survivors.
Methods:
A population survey was carried out in a township near the epicenter of a severe earthquake (7.3 on the Richter scale), which occurred on September 21, 1999.
Earthquake survivors older than 16 years of age were interviewed by trained assistants using the Medical Outcomes Study Short Form-36 (MOS SF-36) and Disaster-Related Psychological Screening Test (DRPST). A total of 1756 respondents were surveyed over the 30-month follow-up period. All the data was analyzed with SPSS 10.0 sofeware.
Result:
At six months and three years after the earthquake, the prevalence of PTSD (cut off point: 3/4) were 23.8% and 4.4% respectively. The residents suffering from PTSD scored lower for each concept from the MOS SF-36 at these two time points. After 3 years post earthquake, progress in the eight concepts and two domains of the MOS SF-36 showed a negative trend for survivors classified as from the following groups: recovering, persistant health problems, persistant PTSD, and PTSD of delayed onset type. Notably,, the PTSD of the delayed onset type exhibited a worse quality of life when the disease occurred.
Conclusions:
Three years after the earthquake, it was clear that the prevalence of PTSD had declined and that the quality of life of the survivors varied with how well their PTSD had progressed.
