摘 要
醫學生在實習醫師的階段中,必須面臨進入「某一個專科」住院醫師的決定,而這個未來執業專科的選擇,關乎日後的醫師專業生涯方向及前途發展。在決定的歷程中,或許有著思考上掙扎與衝突;有各式利害關係的考量與比較;有對未來生涯的期望與不確定;有外在環境給予的期待與壓力;有理想與現實之間的拉扯;……等內心深層之經歷,這些個人經驗,絕對都是獨一無二的。因此本研究希望能夠藉由敘說的方式,來傾聽每一位具有實習醫師身份之研究參與者其專科選擇決定歷程故事,並從中進行詮釋和體會。
本研究之研究方法採質性研究中的「敘說研究法」,以深度訪談的方式來蒐集研究參與者的生命經驗和心路歷程。訪談結束後,將錄音轉謄為逐字稿,輔以參與者檢核的方式進行故事的撰寫及資料分析。
本研究以七位研究參與者「選擇未來執業專科之決定歷程」議題所敘說的內容,找尋呈現的主題與有意義的訊息,最後撰寫出每位研究參與者的決定歷程故事,並分析其中影響個人專科選擇之因素,藉此架構出一套可包含所有研究參與者經驗之「專科選擇順序圖」與「選擇專科之決定歷程模式」。另外也針對本研究中,研究參與者如何面對看待此次敘說經驗進行分析,討論『敘說研究』對於研究參與者的影響所在。
最後,就本研究結論,對於社會大眾、醫學教育與醫務管理提出建議,並給予後續研究者經驗上的建議。
ABSTRACT
In the process of practicing as an intern, student in college of medicine has to choose to be a resident in a certain specialty. And the final decision may strongly relate to their career path in the future. As a result, struggles, conflicts, comparisons among all kinds of advantages and disadvantages, expectations for future careers, pressure form others and negotiations with reality. These individual experiences are both unique and valuable. As a result, the study aims to listen to individual process from each intern with the method of narrative study.
One of the qualitative research methods, “narrative research”, is applied to the study in the way of thorough interview with each participant. Combining the interview records and member-checking, the cases recording and information analysis would be conducted in the study.
The study aims to focus on seven cases concerning the issue of selecting specialty in the future. Meanwhile, in order to sketch a model containing the specialty selecting procedure and decision tree for all participants’ experiences, each of their experience and selecting factors are finely analyzed with the respect of abstracted issues and messages on the basis of their experiences sharing. On the other hand, the opinions from all participants about the method adopted in the study are also included.
Lastly, based on the conclusion, the advices to the future directions for the public, medical education system, medical management and coming researchers would be drawn in the end of the study.
