本研究的目的在探討鎮靜劑propofol對急重病患能量消耗的影響。研究對象為外科加護中心手術後符合全身性發炎反應症候群並使用呼吸器及鎮靜劑propofol的病患共23位,以新陳代謝儀測量儀測量當使用鎮靜劑propofol 36-48小時時以及停用鎮靜劑propofol 8小時、24小時時的能量消耗;結果發現使用鎮靜劑propofol時的能量消耗顯著低於停用鎮靜劑propofol 8小時以及停用24小時時,而停用鎮靜劑8小時與停用24小時之間則無差異。所測得的能量消耗與以Harris-Benedict Equation估計的基礎能量消耗相比,在使用鎮靜劑propofol時為1.05左右,而停用鎮靜劑propofol 8小時以及停用24小時時皆為1.16左右。本研究發現手術後全身性發炎反應症候群病患在使用鎮靜劑propofol時,體溫與能量消耗無相關;但是當停用鎮靜劑propofol後,體溫與能量消耗成正相關。綜合上述結果,在加護病房評估病患能量需求時,需考量鎮靜劑 propofol對降低病患能量消耗的影響。

Abstract
The effect of propofol on energy expenditure was studied in 23 surgically critically ill patients with systemic inflammatory response syndrome ( SIRS ). Energy expenditure measured by indirect calorimetry was determined during patients were sedated by propofol for at least 36-48 hr, and at 8 hr and 24 hr after the discontinuation of sedation. The energy expenditure during sedation period was 1346.7 ± 240.8 kcal/day which was significantly lower than those of without sedation period. The energy expenditure ( 1497.1 ± 308.5 kcal/day ) measured 8 hr after the cessation of sedation and the energy expenditure (1511.4 ± 329.9 kcal/day ) measured 24 hr after the cessation of sedation were not significantly different. No relationship was found between body temperature and energy expenditure during sedation period, whereas positive correlations were found at 8 hr and 24 hr after discontinuation of sedation. Results of this study showed that energy expenditure was lowered in SIRS patients during sedation period in spite of elevated body temperature. Hence, it is important to consider the influence of propofol on energy metabolism when determining the energy requirement of SIRS patients in intensive care units.
