摘要
許多慢性呼吸道發炎的疾病例如:氣喘(Asthma)、慢性阻塞性肺疾病(Chronic Obstructive Pulmonary Disease),這類患者的痰液、支氣管、肺泡灌洗的黏液中,可以發現有許多的嗜伊紅性白血球浸潤,導致組織上皮結構破壞等情況。許多研究證實組織損傷,白血球會從血管內移行到組織間參與發炎反應,同時血小板亦會受到刺激、活化並吸附到白血球,擴大此反應程度;而運動會改變人體血栓調節能力、影響發炎反應,暖身運動能降低運動導致呼吸道氣體交換功能之異常情況。
因此本實驗的目的在探討劇烈運動前有否暖身運動,是否會影響嗜伊紅性白血球與血小板間相互作用與釋放自由基的能力,藉此設計合適的運動處方,以預防嗜伊紅性白血球相關之發炎反應與血栓形成之危機。
本實驗共選取 23 位健康年輕男性,在了解實驗目的與方式後進行測驗,根據其最大攝氧量分別進行劇烈運動(80% VO2max)、輕度運動(40% VO2max)與暖身運動(40% VO2max, 20 min),並利用物理性及化學性刺激,模擬發炎或血管狹窄之病態環境下,嗜伊紅性白血球與血小板凝集之凝集與過氧化物質釋放的相關機制,並了解黏著分子在血栓與發炎反應中所扮演的角色。
實驗結果得知,切應力及化學趨化物會誘發嗜伊紅性白血球與血小板凝集,凝集的產生與過氧化物質 ROS 釋放有關,血小板的參與會增加嗜伊紅性白血球 ROS 的產生,這一連串血栓發炎的過程與黏著分子 Mac-1 (CD11b/CD18)、 P-Selectin 有關,實驗結果證實血小板的參與使得黏著分子的表現會增加,劇烈運動促使嗜伊紅性白血球與血小板,在接受切應力及化學趨化物的刺激下凝集增加、ROS 產生增加、黏著分子表現增加;若在執行劇烈運動前給予 20 分鐘的輕度運動作為暖身,將可減緩血小板與嗜伊紅性白血球交互作用下黏著分子 Mac-1 (CD11b/CD18)、 P-Selectin 的表現,進而影響血栓與發炎的產生。
Abstract
Backgrounds: Heterotypic interaction between eosinophil and platelet causes pulmonary thrombosis and inflammation, leading to acute respiratory distress syndrome. Although vigorous exercise evokes the risk of eosinophil-associated asthma, a warm-up exercise prior to heavy exercise reduces the severity of exercise-induced asthma. This study investigates how warm-up exercise influences the eosinophil-platelet hetero-aggregation mediated by severe exercise. Methods and Results: Mild exercise (ie, 40%VO2max), severe exercise (ie, 80%VO2max) with and without warm-up exercise (ie, 40%VO2max, 20 min) on a bicycle ergometer in 23 sedentary healthy men were performed on three separate occasions. Leukocyte-platelet interactions induced by physical shear force and chemoattractant agonists were measured by flow cytometry. Analytical results showed that first, leukocyte-platelet hetero-aggregation increased in responses to shear stress, as well as inflammatory factors including lipopolysaccharide (LPS), N-formyl-methionyl-leucyl-phenylalanine (fMLP), and phorbol myristate acetate (PMA). Secondly, severe exercise enhanced the reactive oxygen species (ROS) production in leukocyte with platelet whereas the warm-up exercise suppressed the ROS production when severe exercise-induced leukocyte-platelet interaction. Finally, severe exercise increases eosinophils and neutrophils bound to platelets which promoted (CD11b/CD18) Mac-1 and (CD 62P ) P-selectin expression under shear flow and inflammatory conditions. However, these adhesion molecular expressions were suppressed by warm-up exercise. Conclusions: Severe exercise increases eosinophils bound to platelets under shear flow and inflammatory conditions. However, warm-up exercise suppresses the enhancement effect of severe exercise on the heterotypic cell interaction. The finding provides a new insight into the possible protective effects of warm-up exercise against the risk of eosinophil-related thrombosis and inflammation.
