Scant research has investigated causes of interpreting errors other than the interpreter. Therefore, this research displays multiple upstream causes of interpreting errors and proposes ways to secure professional interpreting performance. This research remodels Reason’s (1997, 2000) human error cheese model into the Interpreting Error Model which displays the mode of error occurrence in a simultaneous conference interpreting event. Then, this research places related studies into a literature archive. This research concludes that interpreting errors have many upstream causes. Yet, all layers can launch defense mechanisms to prevent errors. This research proposes the Interpreting Error Model Checklist. This checklist reflects all the variables in an interpreting event and can be used to observe more SI conferences.

Key Words: human errors, interpreting errors, upstream causes, Interpreting Error Model, literature archive
