研究背景:
牙醫師公會全國聯合會推廣貝氏刷牙法多年,目的是希望學童能夠學到確實潔牙的方式,然而目前推廣的刷牙順序與口訣的多次換手,常形成學童的刷牙技巧的困擾,教學者也必須花費大部份的時間釐清,相當耗時費力,不符成本效益。若就潔牙本意來看,若能簡化操作方式又同樣有潔牙效果,會是潔牙推廣更好的選擇。
研究目的:
  本研究針對全聯會的刷牙順序與換手口訣,設計一套同為改良式貝氏刷牙法,適用於國小中年級以上的刷牙順序圖與簡易雙手刷牙的技巧,進行口腔衛生教育介入研究,進而了解兩個方式對學童牙菌斑指數改變的情形。
研究方法:
  研究對象為高雄市一所國民小學中年級,學童共131人,其中實驗組64人(男32人、女32人),對照組為67人(男36人、女31人)。兩組在前測進行問卷調查和口腔檢查。之後進行兩次刷牙技巧指導,實驗組以改良式而對照組則用全聯會版刷牙法,每次40分鐘,介入一週後作後測與二週後進行後後測,記錄牙菌斑指數。用JMP5統計分析。


結果:
  全口牙菌斑指數方面,兩組在前測的平均值沒有差別,但在介入後牙菌斑改變量(±標準差)上,改良式改變了0.09(±0.20)效果顯著地大於對照組-0.01(±0.20)。同時各部位的指數變化方面,在上顎門牙頰側與左方頰側的指數沒有差別;但在下顎門牙舌側,右上後牙舌側及右方轉角處舌側等較難刷的部位,改良式的牙菌斑指數降低的情形,明顯多於全聯會版。
討論:
  改良式的方法在全口牙菌斑指數及刷牙操作困難的部位,在二週的教學時間效果明顯大於全聯會版。改變量最大的部位,是最不易刷的右上後牙舌側,而兩組都採用右手刷。在左方頰側,兩組用不同側手刷,指數並沒有差別。
  因此顯示用哪一手刷並不會影響刷牙效果,四年級學童教雙手刷牙法,沒有問題。但換手次數太多、太頻繁,對學童的學習形成干擾。故建議刷牙的順序與換手圖可改採用改良式貝氏刷牙法,以利推廣潔牙工作。
Background:
Although “Modified Bass method”had been promoted by China Dental Association for many years for schoolchildren who could learn correct method of toothbrushing. But the pattern of change-hand operation is too complicated and hard for children to learn this method. Teachers should make more effort to let children understanding with which hand to brush which part of teeth. It will be better choice if we could simplify the way of change-hand and keeping the same benefit.

Study purposes:
The purposes of this research were to design an improvement of “modified Bass method”, Which can be applied to educational curriculum on 4th grade children of primary school, and to investigate the different result with the standard method.

Methods:
There were 131 schoolchildren of 4th grade from the same primary school at the downtown of Kaohsiung City. The experiment group had 64 schoolchildren, and the control group had 67 schoolchildren.
At the first stage, schoolchildren in both groups finished pre-test dental plaque index, control group had modified Boss method curriculum; the experimental group accepter the“Improved Boss method”intervention .
There were 2 classes (40 min each ) for both groups, and received post-test plaque count record (PCR) . The statistical analyses were performed by JMP5.

Results:
1. It was found that the full mouth score changes on plaque index (0.09±20, P-value=0.0091) from pre- to post-test were statistically significantly better in the experiment group.
2. In the arrears where is easily brushing for example buccal side of upper-central incisor&posterial-left teeth, there was no difference for both groups. But in the area where is not so convenient cleansing, for example lingual side of right-upper posterial teeth and lower incisors. PCR improvement of experiment group were statistically significantly better that the control group.

Conclusion:
The plaque index was improved significantly after fist intervention of the Improved Bass-method. But the modified method even getting worse. It means that Improved method is easier accepted by schoolchildren, it’s more effective toothbrushing method for oral health promotion.
