全民健保自1995年開始實行至今已逾十一年，自2002年全面實施總額支付制度以來，已使我國每人醫療費用成長率控制在3%-4％左右，顯示總額支付制度對醫療費用控制有其成效。但是在2005年4月20日醫界大遊行中，我們看到醫界抗議健保總額制度。從2004年衛生署的統計資料中顯示，台灣國民醫療保健支出（NHE）佔國內生產毛額（GDP）的比率為6.17%，醫界認為此比率與各國相較實屬於低比例的國家。目前總額支付制度實施的癥結在於申請醫療費用點數的成長率遠高於總額增加幅度，造成點值持續滑落。醫療費用就長期而言是必然會成長，在各國醫療費用支出每年持續高漲的同時，為了能適當控制醫療成本，有必要實施總額制度控制醫療費用，但是這也因此造成醫界的困境，醫界建議廢除總額支付制度，希望能另行商議新的配套措施。

從醫界的訴求中，引發本篇論文研究總額支付制度的動機，進而從總額設定的公式中，衍生問題的思考，到底造成醫療費用增加的原因有那些？在目前總額設定公式所考慮的因素之外，是否還有其他的因素應該加入其中？在總額公式設定醫療費用成長率時，是否可事先參考某些總體經濟指標做為醫療費用成長率的依循？使得醫療費用成長率的設定能與總體經濟狀況有相當程度的互動與連結，不至於與實際總體經濟狀況產生太大的脫節。

為了釐清以上問題，本論文主要是剖析影響醫療費用支出成長的因素，進一步結合總額支付制度設定公式，從醫療費用需求面的決定因素中，以因素分析的方法，探討總額支付制度設定公式的非協商因素設置是否適當。但是，是以需求面因素中的所得面向研究之，就所得面向的“因素”與“成長率”兩大方向加以討論。在本論文中，因為依循總額支付制度公式設定之精神，所以對於醫療費用成長供給面的決定因素，將不予考慮。

從總額支付制度設定的公式中可知，造成每年總額增加幅度的高低，除了實際投保的人口數的增減之外，主要還是取決於每人醫療給付費用的成長率，即協商因素與非協商因素成長率的加總。但是協商因素因為部門的不同，而有不同的考量變項。可是對非協商因素而言，各部門呈現相當一致的狀況，這其中包括了自然成長率：反映人口增加、人口老化，即投保人口組成結構改變對於醫療費用的影響率；與醫療服務成本指數改變率：反映醫療服務成本的上漲或下跌。

本論文研究資料引用林小嫻（1994）於台灣經濟學會年會論文集中「國民醫療保健支出之統計」文中，所計算出來1980年至1990年國民醫療保健支出統計資料；與衛生署所公佈的1991年至2004年的「國民醫療保健最終支出統計」，總共二十五年的國民醫療保健支出時間序列資料。但是是以國民醫療保健支出統計資料中保險部門的醫療費用為變項，為了與總額公式中的非協商因素結合探討，去除人口變動（自然成長率）與物價（與醫療服務成本指數改變率）對醫療費用的影響。最後使用平均每人實質醫療費用做為本文醫療費用支出的變項。同理，在所得的變項也是去除人口與物價的影響因素後，引用行政院主計處所公佈，從1980年到2004年的平均每人實質國內生產毛額為所得變項。除此之外，為了得知所得與醫療費用兩變項的成長率之間是否有長期的均衡關係。本文引用1981年至2004年，國民醫療保健支出的保險部門醫療費用年增率，與經濟成長率（實質GDP的年變動率）兩者做實證分析，據此探討兩者年變動率之間是否有長期均衡關係。

在利用Phillips-Perron（PP）單根檢定、Johansen共整合檢定、Granger因果關係檢定、衝擊反應與變異數拆解的實證研究分析中顯示，本論文研究變項皆為非定態，其整合級次皆為I(1)。平均每人實質國內生產毛額與平均每人實質醫療費用兩變數不存在共整合關係，所以就長期趨勢而言，兩者之間並未有共同可預測的關係。在總額支付制度設定的公式中，以因素分析的角度來看，就非協商因素而言，只需包含自然成長率與醫療服務成本指數改變率即可，不需再多加入所得此變項。

但是另方面，不論從Johansen共整合分析、Granger因果關係檢定、衝擊反應與變異數拆解的實證結果分析中，一再顯示經濟成長率與保險部門的醫療費用年增率兩變數之間長期的密切關係。就共整合分析中，經濟成長率與保險部門的醫療費用年增率兩者之間存在共整合關係，這意謂著兩變數之間在長期具有共同可推測的趨勢。從Granger因果關係檢定實證分析中，可知在長期，經濟成長率與保險部門的醫療費用年增率兩變數之間呈現雙向的因果關係，彼此之間存在反饋現象。另外由衝擊反應與變異數拆解研究結果中，顯示保險部門的醫療費用年增率是一內生變數，它並不獨立於經濟成長率，也就是說，保險部門的醫療費用年增率依附著經濟成長率。

從本論文實證結果，可以提供未來政策制定者參考的方向。未來努力的方向可藉由經濟成長率的預估，進而在決定全民健保醫療給付的年成長率時能提供重要的參考資訊，因為此兩者變數在長期具有共同可推測的趨勢，表示一方變數的變動，在未來相當程度上可由另一方變數提供訊息加以預測。所以經濟成長率將是重要的總體經濟指標。這也使得未來總額醫療費用的估算，至少有一總體經濟指標做為參考依循的根據，而不致於偏離總體經濟情況太多，造成醫療費用的浪費與誤判。
