本研究目的是瞭解居家照護失智老年病患之家庭照顧者其自覺準備度及生活品質的情形,並探討其相關因素。於北部某醫學中心之神經內科門診收案,共收集120對失智症老人及其照顧者資料。採用結構式問卷,包括Archbold 的準備度量表及SF-36生活品質量表。
研究結果主要發現:這群失智症病患家庭照顧者有中度的自覺準備度;在取得醫療照顧系統的幫助及感覺照護活動愉悅上,是自覺準備較不足之處。生活品質方面:失智症病患家庭照顧者比罹患其他疾病的照顧者有較差的生活品質,尤其是「因生理健康造成角色限制」、「因情緒問題造成角色限制」、「社交功能」、「活力狀況」及「整體健康感受」方面。照顧者自覺準備度足夠時,則其生活品質之心理健康及整體健康感受分項會較佳,自覺準備度可解釋11.2%心理健康分項總變異量與9%整體健康感受分項總變異量。若病患疾病總數愈少、病患患病年數愈短、家庭照顧者年齡愈年輕、性別為男性、教育程度為專科和大學以上、與病患為非配偶的關係及自覺準備度愈充足者,生活品質愈好。
本研究結果可作為醫護人員在舉辦相關介入措施研究之參考,改善生活品質。
The purpose of this descriptive cross-sectional study was to understand preparedness and quality of life, and their related factors for family caregivers of the dementia patients. A total of 120 family caregivers of patients with dementia from neurological clinics at a medical center in the northern Taiwan participated in the study. Preparedness scale developed by Archbold and Medical Outcome Study(MOS) SF-36 were used to measure preparedness and health-related quality of life (HRQoL), respectively.
Results indicate that family caregivers of patients with dementia in this study had moderate preparedness. Preparedness in getting information from health care system and making caregiving activities pleasant appeared to be poorest among all dimensions of preparedness for these family caregivers. For HRQoL, these caregivers appeared to have poorer health outcomes especially in role-physical, role-emotion, social functioning, vitality, and general health as compared to caregivers of other types of patients. Those family caregivers with better preparedness appeared to have better mental health and general health outcome in this study. Preparedness was able to explain 11.2% of the total variance in “mental health” outcome and 9.0% in“general health”outcome. In addition, less numbers of concomitant diseases, shorter duration of the illness, younger caregiver, male caregiver, caregiver with at least college education, caregiver being other than spouse, and caregiver with better preparedness predicted better HRQoL.
Results of this study can provide a base for future development of interventions for family caregivers of the dementia patients to improve their health related quality of life.
