研究目的：探討台南縣醫院歇業對當地民眾醫療可近性與滿意度之影響。
研究方法：以橫斷式研究法，採類實驗設計，將研究對象分為有醫院歇業地區民眾（實驗組）與沒有醫院歇業地區民眾（對照組）兩組。以台南縣（市）住宅電話號碼簿進行系統抽樣，抽出實驗組與對照組各200位20歲以上之民眾，使用結構性問卷於2003年11月-12月間進行電話訪問，共完訪400份問卷。
研究結果：
1. 在單變量分析時，有醫院歇業地區民眾在門診、急診與住院的服務利用量都比無醫院歇業地區民眾高，但只有門診的服務利用量達到顯著性差異。
2. 在控制外在環境變項及傾向因素、能力因素與需求因素後，發現有醫院歇業地區民眾的醫療服務利用較少，但並未達到統計學上的顯著性差異。
3. 最近3個月有使用門診醫療服務的民眾相較於沒使用的民眾，對整體診所及醫院醫療服務有較高的滿意度，但未達顯著性差異。
4. 醫院歇業地區民眾對診所的整體服務品質滿意度較高，且達顯著性差異；但兩地區民眾對醫院整體服務品質滿意度並未有顯著性差異。
結論：本研究並未發現地區醫院歇業對當地民眾醫療服務利用量產生顯著性的差異。在醫療服務的滿意度上，醫院歇業地區民眾對診所的整體服務品質滿意度較高。所以本研究結論為醫院歇業並未對當地民眾的醫療可近性造成直接影響。

Abstract
Background： Since the launch of National Health Insurance in 1995, the barrier to access to medical services has reduced significantly for people. However, the number of hospitals has decreased gradually according to data released from the Department of Health. In Tainan County, there were three district hospitals closed between 1998 and 2002. According to the Medical Care Network plan, district hospitals provide primary care for local residents. The closure of district hospitals may have a negative impact on access to medical services of local residents.
Objectives: To investigate the impact of hospitals closure in Tainan County on access to medical services of local residents and their satisfactions with medical services.
Methods: A Quasi-experimental design with cross-sectional telephone survey is used. Study subjects are residents with 20 years old and above, randomly selected from areas with hospital closure (experimental group) and areas without hospital closure (control group). The phone books of Tainan City and County are used for the systematically random selection with 200 subjects drawn from each group. The telephone interviews with a structured questionnaire were conducted during November and December of 2003, and a total of 400 residents were surveyed.
Results:
1. Residents in areas with hospital closure use more outpatient, inpatient, and emergency services than residents in areas without hospital closure, but only the use of outpatient services is significantly different statistically.
2. After controlling external environments, predisposing, enabling and need factors, the uses of medical services are less in areas with hospital closure, but the difference is not significant statistically.
3. To medical services provided by clinics or hospitals, the overall satisfactions are higher among subjects with than without the use of outpatient services in the past three months, however the different is not significant.
4. Residents in areas with hospital closure have a significantly higher overall satisfaction with services provided by clinics than residents in areas without hospital closure. But their overall satisfactions with services provided by hospitals are not different.
Conclusion: This study does not found any significant impact of district hospital closure on local residents. The overall satisfactions with medical services provided by clinics are higher among residents in area with than without hospital closure. To sum up, hospitals closure did not affect local residents’ access to medical services directly.
