隨著資訊科技的進步,醫令資訊系統幾乎取代了手寫處方。藉著電腦快速的資料處理能力,部份病人安全也可由醫令系統大加改善。對基層醫療診所而言,限於經費與資訊人力,只能就數種市佔率較高的醫療作業系統套裝軟體擇一使用。以使用者為中心的觀點來看,這些套裝軟體並無法提供快速而即時的維護能力,對藥物相互作用、錯誤處方、健保申報預審等也沒有較友善的人機介面。
全國所有衛生所目前都使用資策會開發之衛生所醫療保健資訊系統,簡稱PHIS,日常維護則由各處醫療資訊網服務中心負責,一般多以S.C.稱之。PHIS雖然會機動依健保局有關規定修改,但時程至少以月計算,對操作上出現的問題的改善也曠時費日。無法滿足對以醫師為使用者的要求或期待。
研究者利用dBase III 開發名稱為Dr-HIS之醫令資訊系統,對於操作上出現之問題可在1-2小時內解決,速度快捷,對病人安全或健保申報預審也有多項功能。利用層級分析方法,以logical decision for window作為決策工具也證明Dr-HIS 相較於PHIS 或健保局之EZ-HIS 都有較佳之表現。
Computerized drug order entry takes the place of Hand written order by degrees.
Under the basis of the rapid data-processing the patient safety and medication error may be improved apparently by computerized drug order entry(CDOE) or computerized physician order entry(CPOE), although packaged software avails the practitioner of some assistance, there is limited use friendly.
PHIS was operated by the health center all over the country and maintained regularly by the health information network service center of DOH. Function of PHIS and the EZ-HIS supported by NHI are lesser compared with packaged software.
We thus develop a CPOE named Dr-HIS. It satisfies the user in health center which is approved by the method of AHP.
