研究問題:目前文獻中瞭解監控-削減威脅訊息傾向與不確定性無法容忍度與健康焦慮都可能與慢性病適應有關,但這幾個變項之間如何作用影響個人的適應,或與健康焦慮及後續個人適應的關係為何,值得進一步的探討。因此,本研究的目的在瞭解監控-削減威脅訊息傾向、不確定感無法容忍度、健康焦慮與慢性病適應的關係,適應相關範疇包含慢性疾病適應功能、憂鬱症狀及焦慮症狀,並以糖尿病及高血壓為例,嘗試瞭解在不同特性的疾病壓力下,各研究變項與疾病適應的關連上是否有所差異。

研究方法:本研究以157位高血壓或糖尿病患者為研究對象,採用立意取樣,以問卷調查的方式進行蒐集資料,問卷內容包含個人基本資料、監控-削減傾向量表、不確定感無法容忍度量表、健康焦慮量表、心理社會疾病適應量表、簡式貝克憂鬱量表、貝克焦慮量表七個部分。將所得資料以SPSS軟體進行統計分析,使用卡方檢驗、t檢驗、單因子變異數分析、皮爾森積差相關及階層迴歸來檢驗研究假設。

研究結果:高血壓與糖尿病患者在研究變項分數表現上並沒有顯著差異。檢證前置變項對適應與情緒影響上發現,不確定感無法容忍度在監控傾向預測健康焦慮時有中介效果;另外健康焦慮在不確定感無法容忍度預測焦慮症狀時有中介效果;至於監控傾向、不確定感無法容忍度及健康焦慮在預測心理社會疾病適應或憂鬱症狀時,未發現有顯著的中介關係,但部分變項具有單獨的預測力,且需特別注意人口變項在顯著的解釋力中有相當的比重。

討論:適應結果包含的心理社會疾病適應、憂鬱症狀及焦慮症狀代表了慢性病患適應的不同層面,所以應該分開討論。研究中值得探討的結果發現有:(1)監控傾向與本研究假設不一,反為適應的有利因子,這可能反應本研究收案族群的特殊性或本國文化因素所造成結果。(2)不確定感無法容忍度與健康焦慮在預測焦慮症狀的路徑中確實扮演中介的角色,因此專業人員在協助此類個案時,減少健康焦慮的認知介入將有利於不確定感無法容忍度高之個案的焦慮症狀。(3)協助個案行為層面的適應時,需注意個案背景環境的特殊性,提供符合其有效資源與需求的協助。(4)預測行為層面的適應或憂鬱症狀,可能仍有其他重要變項未被研究考慮,例如疾病控制感。(5)高血壓與糖尿病患者的適應程度並無顯著差異,需將不同病程時期納入考慮。
