論文摘要
本研究目的在比較遠紅外線與熱敷腧穴治療,對血液透析病人自律神經活性及生活品質之影響。採類實驗性設計,介入組31人,控制組30人。介入組以遠紅外線墊、控制組以熱敷墊為工具,置放於病患任脈穴位上,以每週三次、每次30分鐘、400 C,持續12週的措施。每滿四週抽取病患血液、以心律變異儀收樣,及在介入措施前、後用WHOQoL生活品質量表,檢測病患各項改變及收集病患資料,並用SPSS14.0英文版套裝軟體進行分析。研究結果(1)在血液檢查值上:遠紅外線組病患在白血球、血色素及尿素氮等檢驗值上有統計上的顯著差異,而熱敷墊組則只有血清蛋白質有顯著增加。(2)在心律變異儀的資料顯示:遠紅外線組病患在滿12週時 LF(低頻波)上有顯著的增加(上升,p= .01),而病患心跳次數卻未增加,其功效在增加每次心跳強度主,ANS activity也提高,增加病患自律神經的活性(增加,p= .00),對壓力拮抗能力(Stress resistance)亦有顯著增加效果(p= .02),Fatigue index(疲憊指數)有顯著的降低效果 (p= .02);而熱敷墊組,僅在第8週時PSI值(下降,p= .00)、VLF (極低頻功率)(上升,p= .03)及Stress Index有顯著下降(p= .01)等改變,但在滿12週後,所有結果卻無任何差異。(3)生活品質方面:介入措施後,遠紅外線組病患的心理範疇及環境範疇滿意度有顯著增加(p= .02、p= .00),熱敷墊組病患在環境範疇及健康相關生活品質滿意度的滿意程度有增加(p= .02、p= .01),也就是兩組病患在不同的介入措施下對身體安全及保障、家居環境、健康及社會照顧等,可得性及品質上都有顯著的提高。遠紅外線組病患正面的感覺,思考、學習、記憶及集中注意力,自尊,身體意像及外表及負面感覺等構面有滿意度提高,熱敷墊組則在對本身身體健康、心理健康、獨立程度、社會關係、環境、宗教信仰或信念等多層面的總合滿意度有顯著提高。
The object of this study is to compare the clinical efficacy between far-infrared rays and thermal of the acupuncture point toward the influence of autonomous system (ANS) and quality of life among hemodialytic patients. Totally 31 patients were enrolled into the study group (far-infrared rays ), and another 30 patients receiving heating pad were the control group. Both energy sources were placed upon the conception vessel, 30 minute each time three times a week with the temperature fixed at 40 degree Celcius, for a total of 12 weeks.
Blood sampling, heart rate variability analyzer (HRVA), and WHOQoL (quality of life) measurement were collected every 4 weeks before and after the study. The results showed the far-infrared rays group had statistically significant difference in WBC count, hemogobin, and blood urea nitrogen measurement. While the control group revealed significant increase in serum protein. HRVA study demonstrated significant increase in low frequency (p= .01), heart beat intensity/ ANS (p= .00), and stress resistence (p= .02); and significant decrease in fatique index (p= .02) in the study group. While significant decrease in pressure stress index (PSI, p= .00), and significant increase in very low frequency (VLF, p= .03) were observed at 8th week in the control group; but all the changes diminished at the end of the 12th week. The WHOQoL study showed significant increase in mental and environmental aspects (p= .02, p= .00 respectively) in the study group; and in the control group significant increase were seen in environment (p= .02) and health-related (p= .01) QoL measurement.
In conclusion, both groups showed significant increase in body security, environment, health, and social care QoL measurement. The far-infrared rays group had significant increase in positive thinking, including meditation, learning, memory/ concentration, self-respect, and body image QoL measurement. The heating-pad group exhibited significant increase in physical and mental health, independence, social relationship, environment, and religion QoL study.
