臨床上,病人斷裂的周邊神經在經過顯微手術的接合後,其功能的恢復情況往往不令人滿意。投射到周邊的中樞神經元軸突被截斷後,興奮性毒殺被認為對於細胞有害,因此我們探討顏面神經損傷及修復後,大鼠顏面運動神經元在細胞內的麩胺酸受體次單元表現量的調節。以雄性SD大鼠為實驗動物,將其左側顏面神經截斷再縫合後,分組分別存活一週、二週、四週及十週,而對照組動物只接受神經截斷但不縫合。受傷的顏面運動神經元功能的恢復以下列方式評估,包括觸鬚擺動及眼瞼閉合的觀察,及以肌電圖儀作神經傳導檢查。我們發現在神經縫合之後,損傷的顏面神經會隨著動物存活時間的延長而有明顯功能恢復的現象。此外,相較於對照組有明顯細胞數目的減少,神經縫合避免了受傷顏面運動神經元的死亡。另外,顏面神經截斷及縫合後,顏面運動神經元內的麩胺酸受體一及二的表現量在十週內皆略為下降,但是兩者表現下降的起始時間不同。麩胺酸受體四的表現量則在受傷後二週略為下降,但在十週時則明顯回復。NMDA受體一的免疫反應在所有觀察時間點皆呈稍微下降的情形。過去的研究顯示,投射到周邊的中樞運動神經元軸突被截斷後,細胞內的麩胺酸受體次單元表現會大幅下降。因此我們的實驗結果顯示,神經縫合明顯避免了軸突截斷所造成的麩胺酸受體次單元表現大幅下降情形,然而其生理意義尚待進一步研究。
Even with microsurgical nerve reconnection techniques, functional recovery of the once interrupted peripheral nerve is clinically unsatisfactory. Excitotoxicity is considered to be harmful to the axotomized periphery-projection central neurons. Therefore, we designed to elucidate the regulations of intraneuronal glutamate receptor subunits in repaired rat facial motor neurons. Following left facial nerve cut and suture, male SD rats were grouped to survive for 1, 2, 4, and 10 weeks respectively. The control animals received only nerve transection. The functional recovery of injured facial motor neurons was measured by observation of vibrissae whisking and eye closure and electromyography. We found that following nerve suture, the once injured nerves conspicuously recover their function as the survival time of animals prolonged. Furthermore, the nerve suture prevented the neuronal cell loss which was conspicuous in the control group. In addition, the expressions of both glutamate receptor 1 (GluR1) and GluR2 in repaired facial motor neurons mildly decreased in 10 weeks postlesion, although their onset time were different. GluR4 was downregulated at 2 weeks postlesion but was recovered towards the control level at 10 weeks. Immunoreactivity of N-methyl-D-aspartate receptor 1 (NR1) in repaired facial motor neurons was slightly decreased at all experimental time points. These observations suggest that the nerve suture significantly prevented the dramatic decrease of intraneuronal glutamate receptor subunits which is evident in axotomized periphery-projection central motor neurons. However, the significance is needed to be further investigated.
