暸解護理人員心理變數與病患安全事件通報意願間的關係,可協助醫院管理者提升病患安全,故本研究探討其心理變數(人格特質、專業承諾、工作時心情與對同事能力的負向評價)與病患安全事件通報意願間的關係,及恐懼懲罰是否能提升通報意願。樣本取自北台灣一家大型醫學中心,採橫斷面研究設計與問卷調查法收集資料,共回收有效問卷314份,有效回收率97.5%。研究結果發現通報與他人相關病患安全事件意願低於通報與自己相關病患安全事件意願。再者,通報與自己相關病患安全事件意願與知覺通報效果、依循規定、誠實、專業承諾與工作時的負向心情有正向關係,但與對同事能力負向評價則為負向關係;通報與他人相關病患安全事件意願方面發現與知覺通報效果、依循規定與誠實成正向關係,但與對同事能力負向評價為負向關係,與專業承諾則無明顯關係。最後,護理人員通報與自己相關病患安全事件意願與恐懼被他人通報後伴隨的懲罰成正相關,但與恐懼通報後伴隨懲罰呈負相關。而本研究結果將可協助管理者更有效地配置資源,以進行工作設計與人力招募與訓練,可進一步提升病患安全。
Understanding the relationship between nurse psychological variables and intention to report patient safety events may help managements to improver the patient safety. This study discussed the relationship between nurse psychological variables which involved personality, professional commitment, mood, and the evaluation of coworkers, and also discussed if the fear of report patient safety events will raise the report intention. A cross-sectional and self-administered questionnaire was conducted in one large medical center hospital in north Taiwan. The number of effective returned questionnaires was 314 and the response rate was 97.5%. The result find intention to report others patient safety events was lower than intention to report self patient safety events. The association between intention to report self patient safety events and positive feedback, follow the rules, honest, professional commitment and mood were positive, but was negative with evaluation of coworkers. The association between intention to report other patient safety events and positive feedback, follow the rules, and honest were positive, but was negative with evaluation of coworkers, and no positive relationship with professional commitment. Finally, the association between intention to report self patient safety events and fear of punishment because others report was positive but negative with fear of punishment. The result may help managements to allocate resources effectively, work design, recruitment, and train and then could improvement patient safety.
