本研究旨在建構護理之家住民分類系統，並建立其信效度，檢驗運用在派班後對於病患服務員工作負擔的影響。本研究場所以北部某醫學中心附設護理之家為收案地點，收案的對象為護理之家住民及病患服務員。
本研究採四階段進行，首先參考因素型病患分類系統，以測量病患服務員單次照護活動所需照護時間、其次草擬住民分類表、接著進行本護理之家住民分類表之信效度分析以發展本護理之家住民分類系統，最後實際運用本系統於派班上對於病患服務員工作負擔之影響。經驗證此護理之家住民分類系統，結果發現專家效度之CVI值達.90；住民分類系統之分級具有特異性及敏感性（特異性介於71.4-89％，敏感性介於80-100％）；與日常生活獨立指數ADL及TAI高齡者照護計畫視覺量表具同時效度（r=.502及.737）；評分者間信度，一致性為93-96％；二週再測信度之關係數為1.00。經實際運用在病患服務員派班上，發現可以平均每位病患服務員之工作負荷，提供合宜的工作分配。
本住民分類表具信效度，可以提供作為臨床護理派班之參考。建議可電腦化此護理之家住民分類系統，並定期評估護理之家住民，使可以更快掌握護理之家住民之生活依賴程度之改變及照護需求。且本研究結果運用於病患服務員派班上，可以平均每位病患服務員之工作負擔。

This study aimed to develop a nursing home resident classification system in Taiwan and test its reliability and validity, and then exam the influence of workload of the nurse aid.
There are 4 stages in this study .The first stage is to measure single care time according to factor evaluation patient classification,. The second stage is to construct the resident classification system, according to daily care activity. The third stage is to test its reliability and validity. The result of reliability and validity, CVI（content validity index）>.90；high sensitivity and specificity（sensitivity between 71.4-89％, specificity between 80-100％）；The resident classification has good concurrent validity with TAI and ADL（r=.502 and .737） ；inter-rater reliability （intra-class correlation, ICC）between .78- 1. The fourth stage is staffing, it helped to average each nurse aid workload.
The resident classification system with acceptable reliability and validity can apply to nursing administration. Computerize the classification system to evaluate every resident daily dependence to care is suggested. The result of this study could average workload of every nurse aid, but the workload still high, further research is necessary to aim at t resident intensity for nurse aid to solve this problem.
