腦血管疾病一直是國人十大死亡原因前三名。急性梗塞型中風可於發病3小時內靜脈注射血栓溶解治療法，但大多病患無法於「黃金治療3小時」內趕到醫院，錯失接受治療的良機。因此，如何提早發現、並於3小時內送醫治療一直都是治療急性梗塞型中風努力的關鍵方向。
本研究採質性研究，以深度訪談法探討20位梗塞性腦中風住院病患從發生症狀到就醫過程的行為模式。結果發現整個就醫過程是由症狀詮釋、反應行動及成效評估的循環所合成，而多數病患的就醫過程是由兩個或以上的循環構成。第一次的循環是個人內在的思路，對行動、反應的成效評估是本身或其家屬、朋友認定，第ニ及第三次循環的症狀詮釋及反應行動大多非本人進行。第一次的循環所須時間比第ニ及第三次循環長，第ニ及第三次循環所需時間有縮短之傾向。本研究從既有的就醫行為理論模式歸納整理，並提出一個針對急性梗塞性腦中風病患的就醫行為模式，解釋20位個案從發生症狀到正確就醫的整個過程。分析其中過程，發現就醫時程會因為經過的循環次數愈多而變長，且病患及其家屬對腦中風的詮釋正確與否乃影響就醫時程的關鍵。建議未來應針對中風高危險族群及其家人提供更清楚之教育介入，以減少就醫延遲之發生。

Cerebrovascular disease is one of the most common causes of death in Taiwan. For acute ischemic stroke patients, although intravenous thrombolysis can be used to treat patient onset within three hours, but most of them failed to reach hospital within the ‘Golden three hours’. How to have early detection and seek appropriate medical help within three hours is still the major direction in treating acute ischemic stroke patients.
To explore the contexts and decision-making process of medical seeking behaviors among delayed patients with acute ischemic stroke, twenty in-patients were investigated with in-depth interviews. Based on the interviews, a conceptual model to explain delayed patients’ medical seeking behaviors was formulated. It is composed of continuous cycles in which patients experienced “symptom interpretation”, “action or response” and “effectiveness assessment”. The whole process was formed by two or more cycles. The first cycle was initiated by patients; they or their family would evaluate the initial result from the first cycle and started the second or third cycle. The time needed in the later cycle was shorter than the former. Based on the model and our cases’ behaviors, we concluded that correct interpretation of stroke symptoms plays a key role to patients’ medical seeking behaviors. More patient education focused on high-risk patients and their family was suggested to minimize the possibility of illness delay.
