本研究目的在探討全面性照護計畫改善於心臟衰竭患者自我照顧、生活品質及再住院之成效。採類實驗研究法，以立意取樣在中部某醫學中心之心臟內科病房，選取符合收案標準之心臟衰竭患者60位為樣本，30位為對照組接受醫院常規照護，30位為實驗組接受醫院常規照護及全面性照護計畫介入措施，包括提供心臟衰竭患者健康照護手冊、個別性衛教指導、出院後門診及電話追蹤，並提供諮詢專線。結果顯示實驗組在自我照顧及生活品質的改善程度顯著高於對照組(p＜.001)；實驗組的非計劃性 30天再住院人數、天數及醫療費用顯著低於對照組(p＜.05)。顯示全面性照護計畫可以改善心臟衰竭患者的自我照顧、生活品質，降低非計劃性 30天再住院及醫療費用，期望此照護計劃應用於臨床實務上，提升心臟衰竭患者生活品質。

The purpose of this study was to examine the effectiveness of comprehensive care program on improving self-care, quality of life and decreasing rehospitalizations of patients with congestive heart failure. Quasi-experimental study design was used. Sixty subjects were recruited on purposive sampling from a medical center in Taichung. The control group patients (n=30) received usual care, and the intervention group (n=30) received usual care and comprehensive care program, including healthcare pamphlet, individual health education, outpatient clinic visit and follow up, and phone consultation. The results showed that the improvements on self-care and quality of life were more significant in the intervention group than in control group (p＜.001). Besides, unplanned rehospitalizations in thirty days and payments in the intervention group were significant lower than control group (p＜.05). These results suggest this comprehensive care program has the effectiveness on improving self care, quality of life, lower rehospitalizations and payments. Expecting from this result, comprehensive care program can be used in clinical practice to improve the quality of life of heart failure patients.
