腦傷患者語言障礙個案研究文獻分析之初探
中 文 摘 要
本研究旨在評析腦傷患者個案研究文獻,好的個案研究,應具備哪些要素,本文作者從Yin(1994)指出一篇好的個案研究應具備哪些條件,逐一分析每篇期刊論文的優缺點。
以國外出版的四種期刊,採美國聽語學會的電子期刊資料庫以及SDOL電子資料庫,結合個案研究(case study)和個案報告(case resport)進行搜尋,從中挑選23篇診斷為腦傷的期刊論文進行研究。
研究結果中發現個案研究(1)重視個別差異,針對個案提出適當的介入策略;(2)研究結果可推論到命題;(3)探討臨床上罕見疾病的特徵;(4)以不成熟或是質性研究來研究個案,為發展治療技巧的先驅性研究;(5)能夠觀察個案和治療師、家人的互動;(6)也能探討現象學的因果關係。此外23篇個案研究期刊論文內容中,個案研究最易被評析為不佳的個案研究如下:(1)研究者未清楚的陳述研究問題,須由讀者自行從文章脈絡中找尋其研究問題;(2)通常研究內最易從臨床上科學化檢查達成構念效度,但是內在效度、外在效度卻無法滿足;(3)信度不佳,常因為研究者未將研究步驟一一描述,因此若是後續有讀者想依據同樣的方式進行研究,研究結果定大為不同;(4)個案的基本特徵內,常常未具體陳述研究的起迄時間。
若是能完整將研究問題、研究目的、研究假設、研究設計、個案的基本特徵、研究結果、信效度、資料分析的策略及分析模式完整呈現,就能成為臨床上有價值性的研究。

A Critical Review of Case-Study Literature on Neurogenic Communication Disorders

Abstract

This study was aimed to conduct a critical review of the case-study literature in the area of neurogenic communication disorders. Yin’s (1994) seminal work on the case study methodology provided useful guidelines in analyzing the strengths and weaknesses of the reviewed studies.
Four foreign journals in English were picked as the target source of the literature. With the aid of two e-journal databases (ASHA and SDOS), the author searched and identified 23 most relevant articles that all addressed different issues associated with neurogenic communication disorders.
The results of this critical review identified several characteristics of these case studies: (1) Many studies proposed case-specific intervention strategies. (2) The results were generalized to the theoretical proposition but not some larger population. (3) Some studies described patients with unusual disorders or syndromes. (4) Many studies were at most exploratory in nature, given that they adopted immature measures for description. (5) A few studies made detailed observations of how the patients interacted with the therapists and/or families. (6) Few studies could establish causal-effect relations in the phenomena of concern.
Overall, these studies often fell short of methodological soundness. The common weaknesses are evidenced as follows. (1) Many authors did not explicitly and clearly state the research questions. (2)The internal validity and external validity in many studies were problematic, with construct validity more or less satisfactory. (3) Research reliability was also a big problem, given that the research procedures were often lack of repeatability. (4) Most authors were vague in presenting the timeline of the study.
In sum, this critical review showed that the case-study literature on neurogenic communication disorders was so severely plagued with methodological faults that demand future correction. Despite its clinical and educational utility, future researchers adopting the case-study method should pay more attention to the robustness of their methodology so as to make their results full of scientific merits instead of bugs.
