巴金森氏病(Parkinson’s Disease)；PD是一漸進性慢性退化性的神經疾病，好發於中老年齡的人，年歲越長機會越大，也凸顯巴金森氏病對老年人的威脅。
本研究旨在探討65歲以上PD患者罹病經驗與不同分期的照護需求。採質性研究設計，以立意取樣方式，於北部某醫學中心神經內科門診收案，自民國96年8月至96年11月，運用深度訪談及錄音收集13位PD老年患者罹病經驗，共8位男性、5位女性，受訪者年齡介於65歲至80歲之間，疾病分期涵蓋1-4期。以內容分析法分析訪談資料發現：PD 1-4期罹病經驗共歸納出四個主題：(一)不以為意 (二)不聽使喚 (三)日益退化 (四)深感無奈。其所對應之照護需求為，第一期：應著重疾病相關訊息的提供；第二期：教導患者規則服藥的重要性及支持系統的建立。第三期：照護重點應強調居家環境安全的建構、鼓勵參加病友團體。第四期：醫護人員需協助患者激發生命意義及感受存在價值。本結果有利臨床護理人員照護不同分期患者之參考，協助PD老年患者渡過冗長的疾病生活。

Abstract
Parkinson’s Disease (PD) is a chronic and progressive neurological degenerative disease. It usually occurs among middle age and elderly population, and the incidence rate is especially growing with advanced age. This phenomenon exhibits that PD is a threatening disease for older population.
This study describes the experiences of elderly (65+ yrs) PD patients and the care needs at different stages of the disease. This study was conducted by using qualitative research. Purposive sampling was used to conduct in-depth interviews, which was implemented on neurological outpatient department at a medical center in northern Taiwan. The duration of the interviewing was from August 2007 to November 2007. Thirteen elderly PD patients (8 men and 5 women) aged 65 to 80 with diseases for stages 1 to 4 were interviewed. Interviews were tape recorded, transcribed verbatim and then subjected to content analysis. Four themes emerged from data analysis, including, not taking seriously, not obeying orders, keenly feeling helpless, and feeling keenly helpless. The care needs of elderly PD patients also discussed by each stage. In stageΙthe care should centre on providing disease related information. In stage II, the health professionals should educate patients to take medicine regularly and help them to build the support system. In stage III, the care should emphasize on the safe construction of the daily life at home environment and encourage them to attend the patients groups. In stage IV, the health professionals should help the sufferer to stir up the meaning in life and sense the existing life value. The results of this study can guild nurses to provide appropriate care to PD elderly patients in different stages. Furthermore, it also can assist the patients in getting along well with their suffered lives.
